What Works Against Alzheimer’s?

.Eli Lilly’s promising drug solanezumab failed; so, what works against Alzheimer’s? This drug was supposed to dissolve the amyloid deposits that function like glue and make the patients lose their memory. This phase 3 trial was to test the drug on patients to assess efficacy, effectiveness and safety. But instead it showed that the new drug did not stop the loss of memory.

Brain bleeding as a side effect of potential Alzheimer’s drug

Now all those who were hoping for solanezumab to be effective, will jump on another drug, aducanumab. Biogen from Cambridge, Massachusetts, has developed this drug. Out of 165 subjects only 125 completed preliminary studies. 40 patients who discontinued it, had negative side effects. These included fluid building up in the brain, which was thought to be due to removal of the plaques. But others, had brain bleeding.

Although the drug manufacturer is still hoping that aducanumab will work out as an anti-Alzheimer’s drug, I have my doubts. A drug that can have potential brain bleeding as a side effect does in my opinion not qualify as an anti-Alzheimer’s drug.

Factors that help prevent Alzheimer’s

1. Diet can be as effective as a drug in treating Alzheimer’s

In September 2015 researchers from Rush University published results of putting Alzheimer’s patients on the MIND diet. The MIND diet was a prospective study where 923 people aged 58 to 98 years participated. Researchers followed these people for 4.5 years. Three groups of diets were tested: Mediterranean diet, DASH diet and MIND diet.

The adherence to the diet was measured: those who followed the diet very closely, other participants who were less diligent, and finally those who were not compliant with the diet. With regard to the MIND diet the group with the highest adherence to the diet reduced the rate of Alzheimer’s by 53% compared to the lowest third. This is like a highly effective Alzheimer’s drug! The second group still was able to reduce the rate of Alzheimer’s by 35%, which would be like a regular strength drug. The control diets were the DASH diet and the Mediterranean diet. The group that was strictly adhering to the DASH diet reduced Alzheimer’s by 39%, the group that was very conscientious in adhering to the Mediterranean diet reduced Alzheimer’s by 54%. The middle thirds of both control diets did not show any difference versus the lower thirds.

Findings of Alzheimer prevention by diet

The conclusion was that a strict Mediterranean diet had a very good Alzheimer prevention effect, as did a strict MIND diet. However, when patients did not adhere too well to a diet, the MIND diet was superior still yielding 35% of Alzheimer’s prevention after 4.5 years. The other diets, when not adhered to that well, showed no difference from being on a regular North American diet. Here is more info about the MIND diet.

Conclusion

Avoid the Standard American Diet. Adopt a Mediterranean diet and stick to it in a strict fashion or adopt the MIND diet. The other benefit is that there are no side effects!

2. Stress and Alzheimer’s

2010 study from Gothenburg University, Sweden examined 1462 women aged 38-60 and followed them for 35 years.

Psychological stress ratings went back to 1968,1974 and 1980. 161 females developed dementia (105 of them Alzheimer’s disease, 40 vascular dementia and 16 other forms of dementia). The risk of dementia was higher in those women who had frequent/constant stress in the past. The condition became more severe the more stress they had to face in the past. Women with exposure to stress on one, two or three examinations had higher dementia rates later in life in comparison to women who had no exposure to any significant stress. Specifically, dementia rates were 10% higher after exposure to one stressful episode, 73% higher after two stressful episodes and 151% higher after exposure to three stressful episodes.

Conclusion

Prevention of Alzheimer’s is possible by avoiding stress and seeking counselling when stress occurred .

3. Be creative, prevent Alzheimer’s and dementia

In an April 8, 2015 publication from the Mayo Clinic in Rochester, MN and Scottsdale, AZ 256 participants aged 85 years and older (median age 87.3 years, 62% women and 38% men) were followed for 4.1 years. Psychological tests measured mild cognitive impairment (MCI). At the time of recruitment into the study all of the tests for MCI were normal. As the study progressed it became apparent that there were various risk factors that caused the onset of MCI, which is the immediate precursor of dementia/Alzheimer’s disease.

The finding was that the presence of the genetic marker APOE ε4 allele carried a risk of 1.89-fold to develop MCI and later Alzheimer’s disease.

Further findings of the study

When patient showed signs of depression at the time of enrolment into the study, the risk of MCI development was 1.78-fold. Midlife onset of high blood pressure led to a 2.43-fold increase and a history of vascular disease showed a relationship of 1.13-fold higher MCI development. The good news was that four activities correlated with a lower risk of developing MCI with aging. When the person engaged in artistic activities in midlife or later in life the risk for MCI development 73% lower, involvement in crafts reduced it by 45% and engagement in social activities by 55%. In a surprise finding the use of a computer late in life reduced MCI development by 53%. These are very significant observations. This would be equivalent to highly effective anti-Alzheimer’s drugs.

Conclusion

If you stimulate your mind in older age, even browsing on the computer, this will help you to prevent Alzheimer’s disease.

4. Lifestyle factors contributing to Alzheimer’s

a) Sugar consumption: Sugar consumption and too much starchy food like pasta (which gets metabolized within 30 minutes into sugar) causes oxidization of LDL cholesterol and plaque formation of all the blood vessels including the ones going to the brain. On the long-term this causes memory loss due to a lack of nutrients and oxygen flowing into the brain.

b) Lack of exercise: Lack of exercise is an independent risk factor for the development of Alzheimer’s disease. Exercise increases the blood supply of the brain, strengthens neural connections and leads to growth of neurons, the basic building blocks of the brain. Exercise increases mood-regulating neurotransmitters like serotonin and endorphins.

c) Sleep deprivation leads to memory loss, but so does the use of aspartame, the artificial sweetener of diet sodas. Make your own homemade lemonade. Squeeze the juice of half a lemon. Add mineral water to fill an 8 oz. glass. Add a tiny bit of stevia extract for sweetening. Stir and enjoy. Stevia has been in use for thousands of years.

5. Hormone changes

A lack of testosterone in men and estrogen in women interferes with cognition and memory. For this reason it is important after menopause and andropause (=the male menopause) to replace what is missing with the help of a knowledgeable health professional.

Progesterone is manufactured inside the brain, spinal cord and nerves from its precursor, pregnenolone, but in women it also comes from the ovaries until the point of menopause. The myelin sheaths of nerves requires progesterone and progesterone also has a neuroprotective function. In menopausal women bioidentical progesterone is a part of Alzheimer’s prevention.

Melatonin is a hormone, a powerful antioxidant and a neurotransmitter at the same time. It helps in the initiation of sleep, stimulates the immune system and protects from the toxic effects of cobalt. Lab tests in Alzheimer’s patients found elevated values. In an aging person it is wise to use melatonin at bedtime as a sleep aid and to preserve your brain.

6. Genetic risk of Alzheimer’s

At the 22nd Annual A4M Las Vegas Conference in mid December 2014 Dr. Pamela Smith gave a presentation entitled ”How To Maintain Memory At Any Age”. She pointed out that there are about 5 genes that have been detected that are associated with Alzheimer’s disease and in addition the apolipoprotein E4 (APOE4). About 30% of people carry this gene, yet only about 10% get Alzheimer’s disease, which shows how important lifestyle factors are (in medical circles physicians call this the “epigenetic factors”) to suppress the effect of the APOE4 gene. She also stated that our genes contribute only about 20% to the overall risk of developing Alzheimer’s disease. This leaves us with 80% of Alzheimer’s cases where we can use the brain nutrients and hormones discussed above and exercise to improve brain function.

7. Vitamin D3 protects your brain from Alzheimer’s disease

Alzheimer’s disease is a neurodegenerative disease of old age. We know that it is much more common in patients with type 2 diabetes where insulin levels are high. Studies have shown that Alzheimer’s disease can be termed type 3 diabetes.

The resulting neurofibrillary tangles and amyloid-beta deposits damage nerve cells, which are responsible for the memory loss and the profound personality changes in these patients.

What does vitamin D3 have to do with this?

A 2014 study showed that a low vitamin D level was associated with a high risk of dementia and Alzheimer’s disease.

Specifically, the findings were as follows.

  • Vitamin D level of less than 10 ng/ml: 122% increased risk of Alzheimer’s
  • Vitamin D level 10 to 20 ng/ml: 51% increased risk of Alzheimer’s

The same research group found in two trials that vitamin D deficiency leads to visual memory decline, but not to verbal memory decline.

Generally supplements of vitamin D3 of 5000 IU to 8000 IU are the norm now. But some patients are poor absorbers and they may require 15,000 IU per day. The physician can easily determine what the patient needs in the dosage of vitamin D3 by doing repeat vitamin D blood levels (as 25-hydroxy vitamin D levels). The goal is to reach a level of 50-80 ng/ml. The optimal level with regard to nmol/L is 80 to 200 (according to Rocky Mountain Analytical, Calgary, AB, Canada).

8. Avoid sugar overload

We already mentioned sugar consumption under point 4. But here I am mentioning it again because of the insulin reaction. An overload of refined carbs leads to an overstimulation of the pancreas pouring out insulin. Too much insulin (hyperinsulinemia) causes hormonal disbalance and leads to diabetes type 3, the more modern name for Alzheimer’s. All starch is broken down by amylase into sugar, which means that anybody who consumes starchy food gets a sugar rush as well. Too much sugar in the blood oxidizes LDL cholesterol, which leads to inflammation in the body. The consequence of chronic inflammation are the following conditions: hardening of the arteries, strokes, heart attacks, Alzheimer’s due to brain atrophy, arthritis, Parkinson’s disease and cancer.

Conclusion

In the beginning we learnt about a failed phase 3 trial regarding an anti-Alzheimer’s drug. Next we reviewed several factors that can all lead to Alzheimer’s and that have been researched for many years. It would be foolish to think that we could just swallow a pill and overlook the real causes of Alzheimer’s disease. I believe there will never be a successful pill that can solve the increasing Alzheimer’s problem. It is time that we face the causes of Alzheimer’s. This means cutting down sugar to normalize your insulin levels.

Lifestyle changes necessary to avoid Alzheimer’s

We need to supplement with vitamin D3 because we know that it helps. For women in menopause or men in andropause it is time to replace the missing hormones with bioidentical ones. We need to handle stress and avoid sleep deprivation. And, yes we need to exercise regularly. Following a sensible diet like the Mediterranean diet or the MIND diet makes sense. And let us keep our minds stimulated. Chances are, when we do all of this that we will not need any Alzheimer’s pill. This is not good news for the drug companies, but will be very good news for you. Last but not least, there are no side effects, only health benefits!

Stress Drives Our Lives

Every year the American Psychological Association (APA) monitors the American public how stress drives our lives. This yearly report has been compiled since 2007. About 75% of the people questioned reported that they have experienced moderate to high stress over the past month.

Symptoms when stress drives our lives

What kind of symptoms can stress cause? It can cause sleep deprivation, anxiety, headaches and depression. But there can be more symptoms from any disease that stress may cause. The “Stress in America” report from February 2016 shows on page 5 that unhealthy life habits are used by low-income Americans to cope with stress. A bar graph shows that watching television or movies for more than 2 hours per day is common. Another way of coping is to surf the Internet more often, take more naps or sleep longer. Eating more, drinking alcoholic beverages and smoking more are other unhealthy ways to cope with stress.

As the stressed person gains extra weight and eventually becomes obese, there is a higher rate of diabetes that can develop with all of its complications.

Causes of stress in our lives

The “Stress in America” survey was based on 3,068 adults in the US who completed the survey during August 2015. 72% were stressed out about financial issues. 22% of these said that they were extremely stressed in the past month as a result of money concerns. Other common concerns were work, the economy, family responsibilities and concerns about personal health. Average stress levels among Americans decreased when compared to 2007. On a 10-point stress score respondents rated their stress at 4.9 in 2016 compared to 6.2 in 2007. But according to the American Psychological Association this is much higher than a stress rating of 3.7 considered to be a healthy level.

Stress affects people from all walks of life, workers, women, young adults, students and those with lower incomes.

“Stress is caused by the loss or threat of loss of the personal, social and material resources that are primary to us” Stevan Hobfoll, PhD, a clinical psychologist from Rush University Medical Center said. “So, threat to self, threat to self-esteem, threat to income, threat to employment and threat to our family or our health…” is what causes stress.

Stress drives our lives causing disease

When stress is too much for our system, we are starting to see pathology develop. “Stress is seldom the root cause of disease, but rather interacts with our genetics and our state of our bodies in ways that accelerate disease” professor Hobfoll says. The following are common diseases that can result from chronic stress.

Heart attacks and strokes

In a 2015 Lancet study 603,838 men and women who worked long hours were followed for a mean of about 8 years with respect to heart disease or strokes. All of the subjects were free of heart attacks and strokes when they entered into the study. There were a total of 13% more heart attacks in those who worked extra hours in comparison to those who worked 40 hours per week or less. With respect to strokes there were 33% more strokes in those who worked long hours. Researchers noted a dose-response curve for strokes in groups with various workloads. Compared to standard working hours there were 10% additional strokes for 41-48 working hours, 27% for 49-54 working hours and 33% for 55 or more working hours per week.

Stress drives our lives and causes substance abuse

In order to cope with stress many of us treat daily stress with alcohol. It makes you feel good subjectively, but it can raise your blood pressure causing heart attacks and strokes down the road. A low dose of alcohol may be healthy, but medium and high doses are detrimental to your health.

Next many people still smoke, although scientists have proven long time ago that it is bad for your health. It can cause heart attacks, various cancers and circulatory problems leading to leg amputations.

Overeating is another common problem. Comfort food relieves stress, but it puts on extra pounds. As you know it is easier to put weight on than get it off. Being overweight or being obese has its own problems: arthritis in the hips and knees makes walking more difficult. The metabolic syndrome sets in, which is a characteristic metabolic change causing diabetes, high blood pressure, heart attacks, strokes and certain cancers. The more weight you carry, the less likely you are to exercise. This deteriorates your health outlook.

Diabetes

Stress causes too much cortisol secretion from the adrenal glands. This raises blood sugar, and when chronic can cause diabetes. In addition unhealthy eating habits associated with stress can cause weight gain and high blood sugars leading to diabetes.

In a 2012 California study 148 adult Korean immigrants were examined. They all had elevated blood sugars confirming the diagnosis of type 2 diabetes. They had a  elevated waist/hip ratio.

A high percentage of the study subjects had risk factors for type 2 diabetes. This included being overweight or obese and having high blood glucose readings. 66% of them said that they were feeling stressed, 51% reported feeling anxious, 38% said they were feeling restless, 30% felt nervous and 3% said they were feeling hopeless.

An Australian long-term follow-up study computed risk factors for developing type 2 diabetes. Stress was a major contributor to diabetes.

A 30-day episode of any anxiety disorder had a 1.53-fold risk to cause diabetes. A depressive disorder had a 1.37-fold risk to cause diabetes and posttraumatic stress disorder had a risk of 1.42-fold to cause diabetes.

Infertility

Stress changes hormones in women causing ovulation problems and infertility. 1 in 8 couples in America have problems getting pregnant. Physicians identified stress as at least a contributing factor. But in men stress can also reduce sperm count and semen quality as this study describes.

Remedies for stress

Before you can attempt to remedy stress, you must first detect that you are under stress. You can recognize this when you have problems sleeping, you suffer from fatigue, when overeating or undereating is a problem, and if you feel depressed. Others may feel angry or are irritable. Some bad lifestyle habits may also make you aware that you are under stress. You may smoke or drink more in an attempt to manage stress. Some people abuse drugs.

Here are some suggestions how to remedy stress:

  1. Seek support from family, friends or religious organizations. If you engage in drugs or alcohol overuse or you feel suicidal, it is best to seek the advice from a psychiatrist or psychologist.
  2. Engage in regular exercise. This produces endorphins, the natural “feel-good” brain hormone. This reduces symptoms of depression and improves sleep quality.
  3. Do something that increases pleasure, such as having a meal with friends, starting a hobby or watching a good movie.
  4. Positive self-talk: avoid negative thoughts like “I can’t do this”. Instead say to yourself “I will do the best I can”. Psychologists have developed a technique where they teach patients how to turn negatives into positives. Psychologists call this therapy “cognitive therapy”. You may want to seek the advice of a psychologist to have a few cognitive therapy sessions.
  5. Daily relaxation: you may want to use self-hypnosis, tai-chi exercises or meditation to reduce your stress levels.

Stress is very common. Diverse diseases like heart attacks, strokes, diabetes and Alzheimer’s disease can all be caused by stress. It is important to minimize the impact of stress by seeking family support and support from friends. Engaging in regular exercise will release endorphins and make you feel better. Relaxation exercises and seeking counselling can all help you to manage stress. You cannot ignore or simply tolerate this force in your life. Stress is indeed there, but we can make a difference by managing it to avoid that stress manages us.

High Insulin Levels Can Cause Alzheimer’s

Research published in April 2016 shows that high insulin levels can cause Alzheimer’s. Alzheimer’s disease is more common in diabetics. But until recently nobody knew why there would be this association. Finally new research from New York University (NYU) has shed light on this puzzle. It seems like the key is an enzyme that breaks down insulin, called insulin-degrading enzyme (IDE). Melissa Schilling (no relation to me), an innovation professor at NYU has discovered the metabolic pathway between diabetes and Alzheimer’s disease. This finding has enormous implications regarding the prevention of Alzheimer’s, as I will discuss below. Here is a link to the original paper.

Background information about Alzheimer’s

Alzheimer’s disease affects about 5.2 million Americans and 44 million people worldwide. Above all, there is a progressive loss of cognitive functioning over a long period of time due to senile plaques in the cerebral cortex and the subcortical areas of the brain. These senile plaques consist of amyloid-beta substance and neurofibrillary tangles. This protein material is like glue, which prevents the neurons from working properly and certainly causes memory loss and the confusion, which is so typical for Alzheimer’s patients. Normally amyloid-beta is in solution and prevents lipoproteins in the brain from oxidizing. But when the insulin-degrading enzyme is busy breaking down high levels of insulin, these processes overload this enzyme system. Amyloid-beta experiences supersaturation, as the body does not eliminate it at a normal speed. Consequently, this leads to the glue-like deposits of amyloid-beta in Alzheimer’s brains.

A 2004 estimate for the direct cost of Alzheimer’s disease to the US amounted to  $214 billion. By 2050 this could go up to $1.5 trillion, if there will be no cure Alzheimer’s.

High insulin levels can cause Alzheimer’s, but other mechanisms too

Professor Schilling found in her research that there are four main malfunctions that can lead to high amyloid-beta in the brain of Alzheimer’s patients.

  1. First, with diabetes type1, when the patient does not receive enough insulin, the insulin-degrading enzyme in the brain is not working hard enough. This results in inadequate removal of amyloid-beta from the brain and neurofibrillary tangles of amyloid-beta accumulate.
  2. Second, IDE requires zinc as a co-factor to work properly in breaking down amyloid-beta. Zinc deficiency is quite common, particularly in older people. In this case insulin levels are normal, but the removal of amyloid-beta from the brain is inadequate, as IDE function has become lower.
  3. Furthermore, in early type 2 diabetes there are high insulin levels and there is a competitive inhibition of the elimination of insulin and amylin-beta. This is probably the most common form of getting Alzheimer’s disease.
  4. Finally, excess production of an amyloidogenic protein can lead to an overabundance of amylin-beta, which overwhelms the insulin-degrading enzyme.

What treatment options are there for Alzheimer’s disease?

These four mechanisms from above have several implications as follows.

  1. If a type 1 diabetic patient is insulin deficient, intranasal insulin would be beneficial.
  2. If the patient has type 2 diabetes, intranasal insulin or insulin by injection would be the wrong approach. As stated earlier, there is the competitive inhibition of the elimination of insulin and amylin-beta. It is the insulin-degrading enzyme, which is the limiting factor. This involves simple dietary changes where the patient cuts out sugar and limits starchy foods in the diet. This normalizes insulin levels and the IDE function returns to normal.
  3. Alzheimer’s patients and patients with mild cognitive dysfunction should be tested with glucose tolerance tests (GTT). It the test is abnormal, a knowledgeable dietician should be consulted.
  4. Obesity is strongly associated with hyperinsulinemia and diabetes. Again frequent GTT should be done followed by dietary intervention when abnormal.
  5. Professor Melissa Schilling stated that 86 million Americans are pre-diabetic, but they have no symptoms. Only glucose tolerance testing can diagnose that condition. This will prevent a lot of cases of diabetes and Alzheimer’s disease.
  6. Large parts of the population have no knowledge of the glycemic index of carbohydrates. In order to limit glucose overload and excessive insulin production there is a need for educational nutritional programs. This will be a powerful tool in Alzheimer’s disease prevention.

Conclusion

It has been general knowledge for some time that diabetics have a higher rate of Alzheimer’s disease. People have known Alzheimer’s as “Diabetes of the brain” or “Type 3 Diabetes”. This new research has shed some light on the connection of elevated insulin to Alzheimer’s disease. It was news to me that there is a competitive inhibition of the elimination of insulin and amylin-beta via the insulin-degrading enzyme. It boils down to recognizing that sugar overconsumption causes Alzheimer’s disease. If you want to keep your brain power until a ripe old age, you better eliminate a lot of sugar and adopt a healthy Mediterranean diet.

Vitamin D3 Protects Your Brain

More and more studies are showing that vitamin D3 protects your brain. It protects against MS, but also against Parkinson’s disease and Alzheimer’s disease. In the following I will review what evidence there is to support each of these topics.

Vitamin D3 protects your brain from multiple sclerosis (MS)

It has been known for some time that in the northern hemisphere MS is more common because of the lack of sunshine, which in turn produces less vitamin D3 in the skin.

MS is an autoimmune disease where immune cells attack the lining of nerves. Both nerve cells and immune cells have vitamin D receptors. It appears that immune cells are calmed down by vitamin D3 and remission of an MS relapse is more likely.

There are two forms of MS, the relapsing-remitting MS and the progressive MS. The first one (relapsing-remitting) is more common. After a bout of active MS, the illness calms down and the condition of the patient is stable for some time until the next relapse occurs.

With progressive MS there are two forms, primary progressive MS and secondary progressive MS. The primary form is a case of MS where symptoms steadily worsen, without any remission. The secondary form of progressive MS occurs at the end of fairly stable relapsing-remitting MS. Symptoms become more pronounced and the condition deteriorates steadily from there.

Progression and disability in MS patients with various vitamin D3 levels

Dr. Fitzgerald and colleagues published a study in JAMA Neurology in 2015.

They took 1482 men and women who were on interferon beta-1b treatment. This treatment utilizes the immunomodulator interferon beta-1b and reduces the number of relapses in patients with MS. The study took place between November 2003 and June 2005. Results were analyzed between June 2013 and December 2014. The researchers measured vitamin D levels (as 25-hydroxy vitamin D). The vitamin D levels were obtained at baseline, at 6 months and 12 months.

The number of brain lesions were measured by MRI scans. All of the patients also underwent a functional test, called expanded disability status scale. This measured impairment of ambulation, ability to communicate and activity levels.

Results of this study showed marked differences between patients with high and low vitamin D levels. Those patients who had the highest vitamin D blood levels (more than 40 ng/mL) had the lowest rates of new MS lesions. Previous studies had found that a low blood level of vitamin D (less than 25 ng/mL) in patients was associated with a much higher risk of developing MS. Dr. Fitzgerald’s study showed that a 50.0-nmol/L increase in serum vitamin D levels associated with a 31% lower rate of new MS lesions. Patients with the highest vitamin D level of more than 100 nmol/L had the lowest amount of new MRI lesions (47% less than the patients with the lowest vitamin D levels).

Another study showed that a low-dose vitamin D level accelerated MS. There was a 5.9-fold risk converting the initial relapsing-remitting form of MS into the secondary progressive form of MS.

All these studies show that vitamin D3 can decrease the risk of getting MS. In addition vitamin D3 also delays progression in those who have MS.

Vitamin D3 protects your brain from Parkinson’s disease

Vitamin D3 plays a role in preventing Parkinson’s disease.

Parkinson’s disease is a neurodegenerative disease that causes tremor in muscles, causes balancing problems and eventually can lead to dementia. A metaanalysis was done in 2014 and 7 studies where identified to be relevant. The authors were looking for correlation of vitamin D levels with Parkinson’s disease. The study included 1008 patients in the metaanalysis with 4,536 controls.

  • Patients with a vitamin D level of less than 75 nmol/L had a 1.5-fold higher risk of developing Parkinson’s disease than the controls.
  • Patients with a vitamin D level of less than 50 nmol/L were at a 2.2-fold higher risk of developing Parkinson’s disease.

Another metaanalysis utilized 5,690 Parkinson’s disease patients and 21251 matched controls.

It found that vitamin D levels of less than 20 ng/ml were associated with a risk of 2.08-fold to develop Parkinson’s disease. Interestingly, vitamin D3 supplementation reduced the risk of Parkinson’s disease by 38%. Outdoor work reduced the risk of developing Parkinson’s disease by 28%.

Alzheimer’s disease is a neurodegenerative disease of old age. We know that it is much more common in patients with type 2 diabetes where insulin levels are high. Studies have shown that Alzheimer’s disease can be termed type 3 diabetes.

The resulting neurofibrillary tangles and amyloid-beta deposits damage nerve cells, which are responsible for the memory loss and the profound personality changes in these patients.

What does vitamin D3 have to do with this?

A 2014 study showed that a low vitamin D level was associated with a high risk of dementia and Alzheimer’s disease.

Specifically, the researchers found the following observations.

  • Vitamin D level of less than 10 ng/ml: 122% increased risk of Alzheimer’s
  • Vitamin D level 10 to 20 ng/ml: 51% increased risk of Alzheimer’s

The same research group found in two trials that vitamin D deficiency leads to visual memory decline, but not to verbal memory decline.

Vitamin D3 combined with metformin suppresses cancer

The newest development with respect to vitamin D3 is the finding that it also has anti-cancer effects. Dr. Li demonstrated that vitamin D reduced prostate cancer cell line growth by 45% while metformin alone reduced it by 28%.

But when both vitamin D and metformin were present in the cell cultures there was growth inhibition of 86%. Dr. Li explained that vitamin D potentiated the growth inhibitory effect of metformin.

For years the medical profession stated that 400 IU of vitamin D3 would be enough supplementation. It may be enough to prevent rickets in children. But these low doses will be insufficient in many patients who are deficient for vitamin D to prevent MS, Parkinson’s disease, Alzheimer’s disease or cancer.

A study on medical staff in Northern India showed that 85% of the staff had very low vitamin D levels of less than 10 ng/ml.

It took high doses of vitamin D3 to increase the vitamin D level in the blood.

Generally supplements of vitamin D3 of 5000 IU to 8000 IU are the norm now. But some patients are poor absorbers and they may require 15,000 IU per day. The doctor can determine the patient’s requirement for vitamin D by doing repeat vitamin D blood levels (as 25-hydroxy vitamin D). The goal is to reach a level of 50-80 ng/ml. The optimal level with regard to nmol/L is 80 to 200 (according to Rocky Mountain Analytical, Calgary, AB, Canada).

Many people are deficient with regard to vitamin D, and they do not know it. The most important thing is to do a vitamin D blood test to assess your vitamin D status.

Healing Powers Of Green Tea

Powerful catechins that are a special form of bioflavonoids provide the healing powers of green tea. Researchers have proven that these catechins are only in green tea, not so much in black tea. The most effective of several catechins contained in green tea is EGCG, which stands for EpiGalloCatechin-3-Gallate. It crosses the blood/brain barrier and is very important for the protection of the brain from Alzheimer’s disease. But green tea or green tea extract has a diversified pharmacological action. Researchers said that green tea protects you from cardiovascular disease, from obesity, from diabetes, from autoimmune disorders, from cancer, from Alzheimer’s and dementia.

In the following I like to comment on how green tea or its extract can protect from all of these diseases.

Alzheimer’s disease

Although there are 5 or 6 approved anti-Alzheimer’s drugs, none of them work for very long. They may at best postpone the deteriorating memory for 6 months, but then the effect of the drug wears off. The reason is that the drugs do not stop the production of the deadly beta-amyloid. It is the beta-amyloid that damages nerve cells that you want to preserve so you can think and memorize. In contrast a simple phytochemical, the catechin EGCG has been shown in animal experiments and in human trials to stop beta-amyloid production and increase solubility of beta-amyloid fragments in the brain. The end result is better memory and no further deterioration.

In a study of 13,988 elderly Japanese observed over 3 years the group that consumed 3 to 4 cups of green tea daily had 33% less strokes, cognitive impairment and osteoporosis.

Researchers at the University of Basel, Switzerland enrolled 12 healthy volunteers aged 21 to 28 and fed them extracts of green tea or placebo fluid via feeding tubes. They did this to rule out taste as a factor. The patients underwent functional MRI scans and they also received memory-stimulating tasks. Only the green tea extract was boosting activity in the frontal brain of the subjects. This was located in a specific area, called dorsolateral prefrontal cortex. This area has a connection with language comprehension, reasoning and learning. It also switches short-term memory into long-term memory, called working memory processing.

Healing powers of green tea through new nerve cell development

Researchers showed with animal experiments that green tea extract protects nerve cells from the toxic effect of beta-amyloid. At the same time green tea extract triggers the production of new brain nerve cells (neurons). This is really good news for Alzheimer’s disease patients and their families: green tea extract delays further memory deterioration and stimulates the development of new nerve cells in the brain!

Cardiovascular disease

In a 2006 Japanese study 40,530 Japanese adults aged 40 to 79 years without history of stroke, coronary heart disease, or cancer at baseline were observed for 7 years. Diaries were kept about how many cups of green tea each person was drinking per day. The prevention of heart attacks and strokes was the the biggest effect of green tea extracts.

Men had a mortality reduction of 12% for heart attacks when they drank 5 cups or more of green tea; in women the corresponding mortality reduction for heart attack was 31%, a bigger effect. Overall mortality from strokes was lower than from heart attacks. This made the effect of green tea consumption even more beneficial with respect to stroke prevention. This study did not show any cancer prevention effect for green tea.

Obesity

It appears that green tea increases heat production and burns fat in the process. There was a small effect in terms of weight loss and a beneficial effect increasing the protective HDL cholesterol in this 2012 Polish study on obese patients. The authors compared either 379 mg of green tea extract, or a placebo, daily for 3 months. They concluded: “The results of this study confirm the beneficial effects of green tea extract supplementation on body mass index, lipid profile, and total antioxidant status in patients with obesity.”

Diabetes

Although there are claims in some studies that green tea would prevent diabetes, this question was thoroughly investigated in this Chinese 2014 study.

Researchers did not see any effects on fasting blood sugars or on hemoglobin A1C values. Hemoglobin A1C is a very sensitive indicator for the presence or absence of diabetes. All these lab tests showed no change following consumption of green tea or green tea extract. Forget using green tea for diabetes prevention; cut out sugar and starchy foods instead.

Autoimmune disorders

Sjogren’s syndrome and lupus are both autoimmune diseases. Green tea extract has shown in humans that symptom severity can improve; green tea polyphenols (GTPs) possess anti-inflammatory properties that benefit patients with autoimmune diseases.

In an animal model arthritis researchers determined that T helper cells are weakened and bone resorption is inhibited by EGCG from green tea extract.

Researchers at Harvard Medical School, Boston, MA have noted that green tea extract is useful in calming down the immune response in autoimmune diseases. They concluded: “Altogether, these studies identify and support the use of EGCG as a potential therapeutic agent in preventing and ameliorating T cell-mediated autoimmune diseases.”

Cancer

Many researchers found that EGCG from green tea extract has immune modulatory effects. Furthermore, they saw a positive effect when patients received EGCG in combination with chemotherapy. A combination of cisplatin therapy with green tea extract has been found to have more effects on colorectal cancer and ovarian cancer than each one on its own. Similarly chemotherapy of breast cancer had better results in humans when EGCG from green tea extract was added as an immune modulation. More research, particularly in humans is needed to fully understand the mechanism of action of EGCG.

Toxicity of green tea extract

Animal experiments showed that higher doses of green tea extract could cause toxicity in the liver and in the nose of rats and mice. I was not able to find objective evidence for green tea toxicity in the PubMed system with respect to humans.

Conclusion

Perhaps the most important discovery regarding green tea extract is as follows. It crosses easily through the blood/brain barrier into the brain. This can postpone Alzheimer’s disease and can even lead to new neuron formation. The beneficial cardiovascular effects are also useful and combine well with exercise and good nutrition for prevention. Particularly stroke prevention is a useful property of EGCG from green tea extract. The effect on obesity is marginal whereas there was no effect of green tea on prevention of diabetes. The immune modulatory effect of green tea extract is useful in the treatment of autoimmune diseases and of cancer. Existing treatments for these conditions are becoming more effective by adding green tea extract.

Life Extended By Several Decades

Have you ever thought about the possibility to prolong your “Freshness Date”? At the 23rd Annual World Congress on Anti-Aging Medicine on Dec. 13, 2015 in Las Vegas the endocrinologist, Dr. Thierry Hertoghe from Belgium gave a talk about “How to extend the human lifespan by 40 years”. Dr. Hertoghe explained that it is possible to extend life by paying attention to the factors that prolong life and combining them as an anti-aging type lifestyle. He made a distinction between

  1. normal aging: up to age 82
  2. healthy aging: up to age 100
  3. anti-aging medicine: up to age 122
  4. reversing aging medicine: much more than 122, perhaps to age 150 or more.

Normal aging (up to age 82)

Life expectancy is on average about 82 years. From the age of 50 to 60 onwards you may encounter problems with increased cholesterol, high blood pressure leading to heart attacks and strokes. Coronary artery by-pass surgery may extend an individual’s life by 10 to 15 years. But hardening of the arteries in the general circulation will eventually cut down the blood supply to vital organs leading to premature death that could have been avoided.

Around the mid 60’s to mid 70’s 12.4% of African Americans or 2.9% Caucasians get Alzheimer’s disease. These figures worsen rapidly with further aging: in their mid 70’s to mid 80’s 32.5 % of African Americans and 9.8% of Caucasians suffer from Alzheimer’s disease. At the age of 85+ years 54% of African Americans and 27% of Caucasians have Alzheimer’s disease. With normal aging Alzheimer’s has already increased, and this trend likely is continuing.

Loss of memory, depression and musculoskeletal pain

Memory loss also leads to a shortened survival curve; people with memory loss live two years less on average than compared to a group with no memory loss.

Add to this loss of life because of depression, common in older age. Compared to a non-depressed group over 2 years of older people the depressed group lived 30% shorter.

Musculoskeletal pain in younger age (18-44) was 38%; the next demographic group aged 45-64 reported 61% of musculoskeletal pains; seniors between 65 and 74 had 68% of musculoskeletal pain, and in the demographic group of 75 and up 71% of persons suffered of musculoskeletal pain. As we will learn later there may be hormone deficiencies behind these neck and back pains. If the patient does not seek treatment, this can lead to falls, fractured hips and premature loss of life. Those who survive accidents often become wheel chair bound and end up in nursing homes.

Patients with rheumatoid arthritis and patients with other disabilities have a lower life expectancy

One specific subgroup of patients with musculoskeletal pain are rheumatoid arthritis sufferers. After 10 years of having rheumatoid arthritis patients will have a survival of only about 50%. With involvement of more than 30 joints  (more severe form of the disease) only about 40% will survive. In other words, rheumatoid arthritis is an important factor for lowering people’s life expectancy.

At an age of 65 to 74 men have 23% of disabilities, while woman have 27.5% disabilities. This increases between the ages of 75 or older to 40% for men and 44.5% for women. At the age of 65 disabled men have a 3.5% higher death rate than the average population; disabled women’s death rate is 2.5% higher than the normal population. In other words, disability kills.

Obesity, and heart disease

Urinary urgency and incontinence leads to a 3.13-fold higher mortality rate than a control group of men who do not have these symptoms.

65% of men and 85% of women above the age of 50 have abdominal obesity. This is not just a harmless condition. There is an association between increased triglyceride levels and increased mortality due to cardiovascular disease and diabetes.

By the age of 65-74 heart disease has a frequency of 32% in men and 23% in women. At the age of 75 years and older this jumps to 44% in men and 32% in women. Once the doctor diagnoses heart disease, it causes a lot of premature deaths: an average person with heart disease lives 10 years shorter than those who do not have heart disease!

Healthy aging (up to age 100)

Improving lifestyle factors increases life expectancy

If we look at normal aging, we realize that all these diseases and disabilities we discussed are eventually killing us. In order to live longer we have to take steps that are known to interfere with some of these factors. For instance, quitting smoking will prevent heart disease, several cancers and chronic obstructive lung disease (emphysema). Positive thinking, social support and transcendental meditation will increase survival by preventing mental illness and depression, which in turn will prevent suicides. A healthy diet such as the Mediterranean diet or the Pegan diet will avoid cardiovascular disease and cut down cancer rates.

Live longer with better diet

One dietary change is called the “polymeal”. It consists of fish, fruit, vegetables, garlic, almonds, a moderate amount of wine and dark chocolate. Compared to the Standard American diet this type of diet would add 9 years for men and 8.1 years for women regarding their life expectancy. For instance, prostate cancer showed a 7-fold increase in a group of men who ate a lot of pickled vegetables, fermented soy products, salted fish and preserved meats, when compared to a control group who did not include these foods. In a group of women who had their meat well done and ate three servings of beef per week, breast cancer risk was 4.62-fold higher compared to women who ate meat done rare or medium rare. Overall cancer and cardiovascular mortality dropped by 35% in a study where 5 or more servings of fruit and vegetables were eaten per day.

Regular exercise and supplements of vitamin C and omega-3

A regular exercise program will strengthen the heart and lungs, keep your weight stable, reduce heart attacks and strokes and reduce the probability to develop cancer. A group of men between 61 and 81 were observed over 12 years and divided into those who did not exercise versus those who walked more than 2 miles per day. The exercising men had 19% less mortality compared to the sessile men. Vitamin C from fruit and vegetables or from taking supplements reduces global mortality from all causes by 46% compared to controls that did not. Similarly taking omega-3 fatty acid supplements (fish oil) daily reduced all cause mortality by 20%.

Dr. Hertoghe calls this “healthy aging” and this would allow you to be able to reach an age of about 100 years.

Anti-aging medicine (up to age 122)

Low thyroid hormones

Dr. Hertoghe told the audience that further attention to anti-aging factors could reduce mortality even further. He found over the years that paying attention to correcting hormonal weaknesses would have profound effects on how old a person becomes. Thyroid hormone replacement has been one of the steps that has helped people to feel more energetic, have less muscle pain, less falls, less fractures and complications. It also translates into longer lives.

One slide showed that a low free T3 level (low thyroid) was associated with a 3.6-fold higher death rate. A low free T3 level is an accurate predictor of cumulative death rate in cardiac patients.

T3 is also important for the maintenance of the immune system, which shows in patients with tuberculosis: the one-year mortality rate from TB in thyroid deficient patients was 75%, while patients with a normal thyroid had a mortality from TB of only 7%.

Replacement of missing sex hormones

Secondly, replacing missing sex hormones can add more life because cardiovascular disease is postponed (less heart attacks, less strokes), there is less cancer and better cancer survival, if a person comes down with cancer. Many statistics were quoted.

One interesting slide showed the longitudinal survival follow-up of congenital dwarfs in comparison with their normal brothers or sisters. Untreated male dwarfs turned only 56 years on average, while their unaffected normal brothers turned 75 years on average (19 years longer). With female dwarfs the difference is even more striking: untreated females dwarfs turned 46 years on average, while their normal sisters turned 80 years on average (a difference of 34 years).

Bioidentical hormone treatment prolongs life, lowers heart attack rates and lowers cancer rates

Another publication showed that the heart attack risk was 3.8-fold higher in a group of patients with hypopituitarism (under function of the pituitary gland), but the treatment group (treated with GH) had a normal rate of heart attacks.

11606 men aged 40 to 79 years were followed for between 6 and 10 years. The group who had the top 25% range of testosterone had a 19% lower mortality rates from heart attacks or cancer.

Older women, particularly aged 100 in Okinawa had 2.3-fold higher testosterone levels than women in the US at age 70. On the other hand 70-year old Okinawan women had 2.7-fold higher estrogen levels than US women.

Bioidentical hormone replacement therapy (BHRT) prior to developing breast cancer showed a 27% longer survival among 984 breast cancer patients in Sweden compared to those without prior hormone treatment.

Lower mortality rates for bioidentical hormone replacement therapy of breast cancer patients

In another group of breast cancer patients (2755 patients) aged 35 to 74 who were treated with bioidentical hormone replacement therapy (BHRT) after their breast cancer diagnosis, 50% had a lower recurrence rate (compared to no-BHRT treatment) and there was a reduction of 66% of mortality from breast cancer compared to controls without BHRT treatment. Another study showed that breast cancer patients would have a mortality rate of 33.3% without hormone treatment. After non-estrogen hormone treatment the mortality rate dropped to 12.5% and to 6% after estrogen/progesterone use. This shows the healing results of the various natural hormones.

Treating the cause rather than the symptoms

A group of 280 men and women around the age of 50 were treated with anti-aging hormone replacement for 2 or more years. In the beginning there were 34% of women and 15% of men with coronary artery disease. There were also 36.4% of women and 34.1% of men with high blood pressure. After replacing all of the missing hormones with bioidentical hormones for more than 2 years, coronary artery disease had dropped to 1.6% of the women and 1.08% of the men; high blood pressure had dropped to 2% of the women and 3% of the men. No drugs, just hormones! Of course, initially the doctors prescribed drugs to stabilize their condition, but they could gradually drop them safely. The reason was that the doctors treated the underlying hormone deficiency. The doctors were treating the cause of the cardiovascular disease rather than only the symptoms.

Low mortality of women on bioidentical hormone replacement

Dr. Hertoghe presented data of 6.38-year follow-up of 286 consecutive patients using anti-aging medicine (replacement of missing hormones with bioidentical hormones). These patients had an overall cancer rate of 2.1%, which compared very favorably to the 3.2% cancer rate among US women. The overall cancer rate was  3.1% in French women and 3.1% in Belgium women on no hormones. This is the type of information that is needed following the Women’s Health Initiative (WHI) that scared women into the false belief that hormones would be “poisonous”.

Synthetic hormone do not fit the hormone receptor

In the WHI synthetic hormones caused cancer and heart attacks; the reason for this was that synthetic hormones are not the identical shape as the natural hormones. But hormones and hormone receptors have to fit like a key into a lock; otherwise they are not effective or even block the natural life prolonging action of the natural hormone. This is why in the WHI study the outcomes were poor. Using bioidentical hormones the doctor can prevent heart attacks and strokes and they are also cancer-protective.

Reversing aging medicine (much more than 122, perhaps to age 150 or more)

General medicine has the goal to make patients as healthy as possible. With reversing aging medicine the goal is to make patients as young as possible. They are at their healthiest and feel younger again.

With anti-aging medicine using a healthy diet, exercise and bioidentical hormone replacement therapy the patients can add 15 years of good life. Add to these organ transplants, if necessary, telomerase activators and stem cell therapy. This can add another 25 years of life expectancy to a total of 40 years.

Growth hormone deficiency

Growth hormone deficiency is the one factor that has been underestimated. The discussion of dwarfs in comparison to their healthy brothers and sisters showed us the following. Growth hormone production can add between 19 and 34 years (average 26.5 years) of life. Dr. Hertoghe has done blood tests (IGF-1) and lately also 24-hour urine metabolite tests of growth hormone on aging patients and found that many are deficient with regard to GH production. These were patients where Dr. Hertoghe already replaced their thyroid hormones, if abnormal and replaced their sex hormones when they were low. But they lost hair, developed old looking faces with wrinkles. In addition, a loss of subcutaneous fatty tissue is giving the face a hollow appearance. They also had muscle and joint pains and thin skin, particularly over the back of their hands.

Replacement of growth hormone

He replaced their missing GH using daily GH self-injection with a tiny needle (similar to diabetes injections). Within 1.5 to 3 years the wrinkles disappeared, the faces started to look younger and patients did feel younger. Their muscle and joint pains had disappeared and their hair grew back. The dosage range is between 0.1mg and 0.3mg, a tiny amount of GH daily. This is not inexpensive, but some health care plans pay for this, as a lack of GH is a true hormone deficiency.

About organ transplants

Often it is a single limiting organ that determines when we die, typically the heart, lungs, brain, liver, kidneys, small bowel, pancreas or bone marrow. Organ transplants can add years of life, but it can be cumbersome to find a suitable donor. One study showed that only 40% to 60% of organ transplants are surviving 8 years after the surgery.

Stem cell therapies are other ways to prolong life. More research will perfect this, but essentially stem cells can provide 220 different cell types for in-vitro organ culture. This can probably be of use in the future to replace malfunctioning organs.

The dream of staying younger for longer can be a reality today. You just need to be willing to discipline yourself and watch what you are eating (Mediterranean type diet). Also, exercise regularly and have a positive psychological attitude. If the outdoor air is poor where you live, you may want to consider moving. Move to a place with good air quality. Sleep well for 7 ½ hours every night and retire not later than 10 to 11PM. You need to be asleep between midnight and 3AM as the growth hormone peak occurs at that time.

Take supplements

Take supplements that contain longevity micronutrients (magnesium, vitamins A, C, D, E, B6, B12, Co-Q-10, selenium, zinc, iron in premenopausal women etc.). Replace all missing hormones with bioidentical ones, like thyroid hormones (T3 and T4), sex hormones, DHEA and GH. Stem cell therapy and telomerase activators for cell rejuvenation will also have more of a place in the future.

Even, if you do only part of this reversing aging program you will slow down aging.

Diet And Brain Health

The fact that the topic of diet and brain health keeps popping up in the medical literature, is significant. This year has not been any exception.

The Mediterranean diet in particular has been shown to have very positive effects on postponing Alzheimer’s disease by as much as 5 years.

A clinical study on 674 elderly patients (mean age 80.1 years) without dementia, was published in the journal “Neurology”. It examined the question whether adherence to the Mediterranean diet would affect the degree of brain atrophy. Researchers already knew that Alzheimer’s disease was less common on a Mediterranean diet.

The findings were interesting: a high adherence to the Mediterranean diet led to a higher total brain volume, total grey matter volume and total white matter volume as measured with high-resolution structural MRI scans.

Quality of diet influences cortical thickness of brain

Lower meat intake led to higher brain volume. In addition, more fish intake also caused the mean cortical thickness of the brain to increase. Parts of the brain in Alzheimer’s patients showed atrophy like in the cingulate cortex, parietal lobe, temporal lobe, and hippocampus with good volumes on MRI scans when patients adhered to the Mediterranean diet. These volumes started to shrink when the diet was poor.

Those patients adhering to a Mediterranean diet have brains that on MRI scan look 5 years younger and are much less likely to develop Alzheimer’s disease. Physicians have known for a long time that people, who eat a healthy diet, exercise regularly, don’t smoke and who keep mentally stimulated will generally have healthier brains than people who don’t do these things.

What is the Mediterranean diet?

It involves eating meals derived from plants: vegetables, fruit, cereals, beans and nuts. You can eat fish and poultry twice per week. You cut down the amount of meat and dairy you eat, but you can have a glass of wine per day. Instead of butter olive oil is used instead. Here is more information of what is included in the Mediterranean diet.

Because there is less fat and less high glycemic index carbs in this diet, it is also a diet that lends itself for weight management. You shed a few pounds and reach your ideal body mass index without paying much attention to it.

Apart from the Mediterranean diet the MIND diet has also been shown to prevent brain atrophy. This diet is a combination of the DASH, which physicians developed for controlling high blood pressure, and the Mediterranean diet.

The Mediterranean diet makes you live longer

The Nurses’ Health Study that has been going on since 1976 showed that telomeres, the caps on chromosomes, were getting shorter in nurses who lived on junk foods, but surprisingly nurses on the Mediterranean diet preserved their telomeres. Longer telomeres have an association with slower aging. And people with longer telomeres reach an older age without diseases like heart attacks, liver disease or cancer.

Exercise on top of the Mediterranean diet

In addition to relying on a healthy diet, like the Mediterranean diet, for your health, think about doing regular exercises. These two ingredients together will prevent heart attacks, strokes and other diseases. When you combine exercise with a healthy diet your abdominal girth shrinks as this study showed.

Another study showed that when a Mediterranean type diet is combined with regular exercise, adult onset diabetes occurrence could be reduced by 28-59%.

This is quite a significant effect of two simple interventions: a healthy diet and regular exercise.

Don’t smoke

It does not make sense to go on a healthy diet, exercise and then smoke! Interestingly an Iranian study showed that when people became health conscious, adopted a healthy diet and exercised, they also started to quit smoking. People who did all of this, quit smoking, eating healthy and exercising regularly, were also the happiest and most content.

Exercise your brain

The evidence shows that any stimulation of brain activity, particularly anything that requires active and abstract thinking will protect the brain from developing Alzheimer’s disease.

Another study showed that prevention of Alzheimer’s disease is achieved by quitting smoking, treating high blood pressure, stimulating the brain and treating diabetes.

us. This approach will be the most successful way to prevent Alzheimer’s disease. It starts with quitting smoking. It goes on to starting a Mediterranean diet and staying on it. Regular exercise will take care of preventing heart attacks and strokes. But exercise also ensures that all of your brain cells continue to get oxygen and nutrients. This in turn prevents brain shrinkage.

Weight loss included in Mediterranean diet

The Mediterranean diet has a lower calorie content than the Standard American diet, there will be weight loss. The weight loss will continue until you reach your ideal body mass index. You can stimulate your brain by actively doing computer work, doing puzzles, playing a music instrument and phoning friends. In addition you may want to read reading books etc.. All this  will contribute to preventing Alzheimer’s. Watching TV or movies is not an active mental activity. This is passive thinking, which means it is not as valuable as the other activities. Pick a hobby that enhances your life, and your brain will thank you for it too!

Testosterone

One of the driving hormones in a man is testosterone. We also know that with age this hormone level falls. The lesser known fact is the importance of monitoring testosterone levels in aging males. This way they have the choice of intervening with the aging process. Here are the facts about the male hormone and about replacement of it when it is low. I will also discuss the anxieties of the medical profession to deal with this. Some feel uncomfortable about hormone replacements.

Androgen receptors contained in key tissues

Androgen receptors are situated in the key organs like the brain, heart, muscles, bones, kidneys, fat cells, genitals, hair follicles and skin. They respond to all male hormones, called androgens, like testosterone, dihydrotestosterone (DHT) and DHEA. DHT is produced by metabolizing testosterone with the help of an enzyme, called 5α-reductase in the adrenal glands. This is responsible for hair loss in males and some females. There is a genetic factor for this. It is important that the man continues to have all tissues stimulated by his male hormones when he ages or the key organs mentioned are going to suffer.

A lack of the male hormone as the man ages (around 55 to 65) leads to a slowdown in thinking, osteoporosis in the bones, muscle atrophy (melting in of muscle tissue), and a lack of sex drive. Mood swings can turn the male into the “grumpy old man”. The skin gets thinned and is more brittle.

Animal experiments have shown that the development of fatty streaks in blood vessels happens at a higher rate in castrated animals. The more encouraging finding in these animals is the fact that this condition is reversible by replacement of the male hormone. In healthy males of a younger age all organs are working well. The problems starts when males age and the hormone regulation in the brain slows down, which ultimately leads to andropause in males, the equivalent of menopause in women. When the physician replaces testosterone in an aging man who has low testosterone levels, the stimulation of androgen receptors in key organs, which I mentioned before return organ function to normal.

Reluctance of physicians to prescribe testosterone

In the past medical students were taught that testosterone is causing prostate cancer. This was based on old observations by Dr. Huggins, a Canadian born surgeon who practiced in Chicago, that orchiectomy improved the survival of advanced prostate cancer patients by a small percentage. Dr. Lee pointed out that Dr. Huggins neglected to realize that testicles make both the male hormone and small amounts of estrogen.

History of hormone ablation therapy for prostate cancer

When an orchiectomy was done (because of the belief that the male hormone production was the culprit) inadvertently the real cause of prostate cancer (an estrogen surplus) was also removed thus improving the survival of these patients somewhat. Nowadays we have more sophisticated testing methods. Dr. Abraham Morgentaler (Ref. 1) has compiled a lot of evidence about the importance of testosterone in men. He proved, based on a lot more modern references that it is not testosterone that is the cause of prostate cancer. We know now that estrogen dominance is responsible for prostate cancer and that this develops as stated above because of the low testosterone and low progesterone during the male menopause (also called “andropause”).

Rebuttal of Dr. Huggins’ research

Dr. Morgentaler, a urologist from Harvard University has taken prostate cancer patients and put them on testosterone. To his and everyone else’s surprise testosterone treated prostate cancer patients improved, their prostate cancer either disappeared or become much less aggressive, which can be measured with the Gleason score based on its microscopic appearance. The result was that they did better, not worse on male hormone replacement.

Older physicians still hold on to the belief that testosterone would make prostate cancer worse

Unfortunately the history of male hormone production, orchiectomy and prostate cancer as explained led to confusion among the medical profession. We now know that testosterone is innocent with respect to prostate cancer, testicular cancer or any other cancer. But some of the old-timers among the physicians doggedly hold on to their false belief from the past. If a man asks one of these physicians for testosterone replacement he may not only be told that he/she could not do that, but will also receive a tirade of false statements about testosterone.

No blood clots with male hormone replacement

We dealt with the myth of prostate cancer that is not related to treatment with the male hormone. There is another myth that older physicians often cite: that testosterone would supposedly be causing blood clots. At the University of Texas Medical Branch at Galveston (Texas, USA) a large study was done involving 30,572 men, ages 40 years and older. All had venous thromboembolism and received an anticoagulant drug or an intravascular vena cava filter following their diagnosis. Because they also had a low testosterone level physicians gave them testosterone replacement therapy. In addition, they followed them and monitored them for further venous thromboembolism. None were found in any of the men. The conclusion of the investigators was that filling a testosterone prescription was not associated with any clotting condition.

Aging and testosterone

A longitudinal study showed that testosterone has been declining in the male population over a period of 20 years. Partially there is a relationship to aging. Otherwise estrogen-like substances or xenoestrogens, which are environmental factors, contribute to it as well. Although age is a factor, there is so much variation from man to man, that it is best to just measure testosterone and determine whether the mail hormone level is above or below 500 ng/dL. This seems to be the most reliable indicator in determining whether a man needs hormone replacement, apart from symptoms due to testosterone loss. These are: increased risks for prostate problems and/or cancer, cardiovascular disease, loss of bone density, a rise in cholesterol and urinary dysfunction. Dr. Randolph describes this in detail and also discusses who needs bioidentical testosterone replacement.

The aging male and hormone replacement

A New England Journal of Medicine study from September 2013 explained that apart from testosterone the male body needs a small amount of estradiol, the female hormone. The enzyme aromatase within the fatty tissue achieves this. But the physician must prescribe testosterone replacement as the bioidentical testosterone, so that a small amount of it converts into estradiol by the action of aromatase. Synthetic male hormone compounds with chemical side chains do not provide this small estrogen metabolism.

After a review of the hard facts about prostate cancer risk, it is now clear that older men get prostate cancer because testosterone in their blood is low and their body weight elevation. The extra fat converts androgens by the aromatase into estradiol. This leads to estrogen dominance. This causes breast cancer and uterine cancer in women, prostate cancer in men. When the total testosterone level in a man is lower than 500 ng/dL it is a sign that he needs male hormones to protect his prostate from prostate cancer.

Cardiovascular disease and testosterone

The cardiovascular system has a lot of androgen receptors on its cell surfaces. It is important that the man continues to have the proper stimulus from androgenic hormones (testosterone, dihydrotestosterone and DHEA). This will allow him to have proper contractility of heart cells. It will also allow for relaxation of smooth muscle cells in the arteries to control blood pressure. With a lack of male hormones there is hardening of the arteries, loss of muscle cells in the heart muscle and increase of blood pressure. So far researchers have only noticed an association of low testosterone with diabetes, high blood pressure and heart attacks. It has not been proven that it is the cause (so webmd.com says). But careful replacement with bioidentical testosterone helps patients to get rid of their symptoms, have the energy to exercise and feel better.

Is it safe to replace missing hormones in a male?

Long-term studies have already shown that hormone replacement saves lives, but the medical profession is slow to accept this (Ref.1). Here is a link that explains this a bit further.

If a man who is low in male hormones wonders whether it would be worthwhile to go on testosterone therapy, here is the clear answer: would you like to have a 47% lowered risk of dying, a reduction of 18% in heart attacks and 30% reduction in the risk for a stroke? This is what a 14-year follow-up study published in the European Heart Journal in August, 2015 found.

The same is true for cardiovascular disease as stated above: if the total testosterone level in a man is lower than 500 ng/dL it is a sign that he needs testosterone replacement therapy to protect his cardiovascular system to prevent heart attacks and strokes.

Loss of bone density with a lack of testosterone

Older men can get osteoporosis, which can lead to compression fractures in the spine. In addition fractures can also occur in the hip, the ankle or wrist. Current thinking is that with the lack of male hormones there is also a lack of estradiol via the aromatase pathway in fatty tissue. This small amount of estradiol is able to prevent osteoporosis all his life until testosterone drops with older age. Once again it is important to monitor his male hormone level and replace with bioidentical testosterone when it is lower than 500 ng/dL.

Rise in cholesterol

With obesity there is a metabolic change, called metabolic syndrome. This leads to an elevation of LDL cholesterol, which is a direct risk for hardening of the arteries. In an obese older man with low male hormones there is a double risk. First, there is the low testosterone and secondly the metabolic syndrome, if he is overweight or obese. As a result the heart attack and stroke rates in obese men with low testosterone are much higher. In obese men with normal male hormone levels there are less heart attacks and strokes. Men with obesity need to lose weight by changing their diet to healthier eating habits and starting a regular exercise program with swimming and walking. At the same time those with a testosterone level of lower than 500 ng/dL should have bioidentical testosterone replacement.

Urinary dysfunction

A hyperactive bladder, dribbling, hesitancy and leaking bladder can all be part of male hormone deficiency. But this is not that easy to diagnose. A full consultation by a urologist may be necessary to assess various other causes that could hide behind these symptoms. Part of the work-up though is to measure the total testosterone level and replace what is missing.

Alzheimer’s disease

Alzheimer’s disease can be due to a lack of male hormones. It is therefore important to measure the total testosterone level in a man. If it is lower than 500 ng/dL, as mentioned before , it means he needs male hormone replacement therapy. This will prevent Alzheimer’s disease.

Burnout

According to Dr. Thierry Hertoghe, an endocrinologist from Belgium, there are several hormones that can be missing in a person with burnout: a lack of cortisol, thyroid, growth hormone, testosterone/estrogen, progesterone and oxytocin. The middle-aged manager with burnout would have other hormones missing apart from testosterone. The physician needs to order blood tests to measure whether other hormones are missing. Whatever is low needs replacement with bioidentical hormones.

Some details regarding testosterone measurements and delivery

The deeper you delve into male hormone replacement, the more details there are to consider. First, there is a sex hormone-binding globulin that is mostly produced by the liver and circulating in the blood. It is like a storage form of testosterone and only 1 to 2% of the total testosterone is unbound. This is called the free or bioavailable testosterone. Some physicians measure just that portion of testosterone.

Hormone replacement in a man

Second, when it comes to replacement of testosterone in a man who is deficient for testosterone, there are several delivery systems, which some people find a little confusing. There are testosterone gels with less than optimal absorption. Another application are creams that compounding pharmacies provide. The patient absorbs these creams usually quite well. But some men do not absorb either creams or gels. They need testosterone injections or testosterone pellets. The goal is to replace testosterone in a manner that there is a fairly equal amount of testosterone available circulating in the blood at all times. Some men achieve that only with testosterone pellets, others with testosterone cypionate injections. For this reason blood test that determine the levels of free testosterone are necessary at intervals of about 3 months.

Conclusion

Testosterone is a key hormone in the male that requires monitoring, particularly when he is aging. A knowledgeable physician or naturopath needs to take a careful history of his symptoms. If blood tests show that the total testosterone is less than 500 ng/dL the physician needs to replace with bioidentical testosterone.

Sugar As White Death

We can think of sugar as white death, as sugar is responsible for heart attacks, strokes and even some cancers. Sugar is causing disability and mortality to a much higher degree than most people are aware of: a recent study pointed out that worldwide 184,000 deaths per year are attributable to sugar-sweetened beverage consumption; among those there are 133,000 deaths from diabetes, 45,000 deaths from cardiovascular disease and 6,450 deaths from cancers. Those people who developed disabilities from strokes, heart attacks, osteoporosis and severe arthritis measured 8.5 million disability-adjusted life years throughout the world and were related to sugar-sweetened beverages. 4.5% of these were from diabetes that was related to sugar-sweetened beverages.

These statistics are only regarding sugar-sweetened beverages! This does not take into account mortality from sugar in processed foods, in cookies, cakes and candies. It also does not take into account starchy foods like pasta, bread, bagels, white rice etc. that are all digested by amylase in the mouth and in the gut to turn into sugar within half an hour of ingesting them.

In the following I like to give an overview of what sugar does to our system.

History of sugar production

The initial production of refined sugar was developed in India as this review of the history of sugar shows.

In Great Britain the consumption of sugar was 4 pounds per person per year in 1700; it rose to 18 pounds per year in 1800, to 36 pounds per year by 1850 and over 100 pounds per year by the twentieth century. Similar figures are true for the US and in all developed countries. In 1747 the German chemist Andreas Marggraf identified sucrose in beet root. Since then technology was developed to extract sugar from the beet root, which was cheaper to do than extracting it from sugar cane. Both methods are in use today. In addition high-fructose corn syrup has been developed in 1970 and it replaces sugar in many uses, for instance in soft drinks and in processed foods.

Overall we are exposed to sugar in all disguises, such as sugar-sweetened beverages, candies, sweetened yogurt that is sold as “healthy”, power bars that are sugar laden and many more.

Effect of sugar on our bodies

1. Diabetes

As already pointed out above the average sugar consumption has increased from 4 pounds per person in 1700 to above 100 pounds per person in our time. Our poor pancreas has to cope with this additional burden of sugar and if it can’t, we get diabetes. The CDC says that in 2008 there were 8 cases of new type 2 diabetes cases per 1000 people in the US. The CDC projects that in 2050 this number will likely increase to 15 new cases of type 2 diabetes per 1000 people.

This shows you that the capacity of the human pancreas is limited. There is a breaking point regarding our insulin production. The insulin production has a limit, because the insulin producing cells in the pancreas can only produce a limited amount of this sugar-clearing hormone. When this point is reached the person is said to have developed diabetes. Diabetes causes heart attacks, strokes, kidney failure, blindness and circulation problems in the legs leading to amputations.

2. Cardiovascular disease

Diabetes is not the only problem that sugar causes. Our cardiovascular system is suffering because sugar makes the liver produce more LDL cholesterol that gets oxidized by sugar; the triglycerides are rising as well with continued sugar intake and with too much sugar intake there is excessive weight accumulation causing type 2 diabetes. This leads to more lipids in the arterial walls, called arteriosclerosis. The end results are heart attacks and strokes.

3. Cancer

It may not be obvious how sugar intake can lead to cancer. But sugar has been found to oxidize tissues and in the process produce dangerous free radicals. This causes chronic inflammation leading to mutations in the DNA of cells and weakening of the immune system. This will in time lead to cancer. Many cancer researchers have investigated this in detail in the last decades. I reviewed this in this blog, if you would like more information about it.

4. Brain atrophy and Alzheimer’s disease

Sugar overconsumption has been found to be one important factor in the development of Alzheimer’s disease, which is associated with the development of brain atrophy. Brain atrophy is just the mirror lesion in the brain that comes from hardening of the arteries. Brain atrophy develops when not enough nutrients and oxygen reach your brain cells. Part of the brain surface dies off and memory cells are lost. The end result is dementia or Alzheimer’s. Read more about this here.

5. Arthritis can come from sugar overconsumption

As I have summarized in this blog arthritis often is due to over consumption of processed foods including sugar products.

As I am explaining in this blog Dr. Hoffer has developed a simple supplementation for arthritis that will reverse the metabolic changes that are associated with arthritis. But you must switch to a Mediterranean diet without sugar and starchy foods, if you want to experience relief from your arthritis symptoms.

6. Low fat diet not helpful to reduce heart attack rates

As I pointed out before the low fat diet that was popular in the 1980’s until the early 2000’s did not help reducing heart attacks.

The low fat diet was laden with sugar, meaning that it was a low fat, high carb diet, and all of the problems I described above with weight gain, high LDL, high triglycerides and the development of diabetes caused more heart attacks and strokes. The real solution to preventing obesity and lowering heart attacks and strokes is to use a low carb/low to medium fat diet like the Mediterranean diet.

7. We need our muscles in older age

When we eat too many carbs from chocolate, candy, donuts and pasta there is not enough quality protein in our food to feed our muscles. If this is combined with a lack of exercise we are in double trouble of having flaccid muscles. This leads to falls and fractures, but is entirely preventable by eating a proper diet and exercising regularly.

8. ADHD can be fuelled by sugar

I have reviewed ADHD (attention deficit hyperactivity disorder) and mentioned that sugar and gluten sensitivity may be part of the problem. It is important to sort out nutritional factors by going through an elimination diet. Often our Western style diet (sugar and fat rich) is making things worse for the child with ADHD.

Apart from other measures avoiding sugar is very important for the ADHD patient.

9. Chronic inflammation

What causes chronic inflammation in the body and is responsible for both hardening of the arteries as well as arthritis? If you guessed sugar intake, you guessed right. I explained this in detail in this blog.

This concept is one of the biggest new things in the 21st century. The research goes back to the mid-nineties and culminated in the detection of an inflammatory marker, the C-reactive protein (CRP). The CRP level can now be used as a readily available blood test to detect inflammation in the body. Often this test will be positive in patients with arthritis, autoimmune diseases and cancer.

10. Obesity

Processed food contains wheat and sugar. The problem is that the high gliadin concentration in the Clearfield variety of wheat makes people addicted to food and sugar makes them gain weight. This is the cause of the obesity and diabetes wave. The remedy is to cut out all wheat and sugar as well as starchy foods. Switch to a Mediterranean diet without sugar and starchy foods.

11. Pimples and acne

Who would have thought that acne could come from a combination of sugar and milk products? Careful epidemiological studies have shown that in some regions of Africa, Brazil and Japan teenagers who eat the local food do not get acne, but when they switch to a Western style diet they come down with acne.

12. Tooth decay from too much sugar

Gum infections and severe tooth decay were found in the 1990’s to cause inflammation in the blood, which can be measured by using the C-reactive protein (CRP). Streptococcus viridans, a bacterium that populates gums and teeth can cause subacute endocarditis, a dangerous infectious disease of the heart valves, which can be responsible for sudden death in younger persons. There are other bacteria in the mouth that feed on sugar that we eat, particularly if we do not brush and floss our teeth regularly. This means there is double trouble: Sugar causes cavities and gum disease, but also causes heart attacks and heart valve infections.

Conclusion

Sugar and starchy food consumption affect so many organ systems that it is no wonder that people say that they feel better when they switch from the standard American diet to a Mediterranean type diet. We do not really want to buy a heart attack, a stroke, diabetes and Alzheimer’s disease when we go to the grocery store. But this is what you get in time when you buy the starchy foods and sugar containing processed foods that are in the center part of the grocery store.

The neighbor’s shopping cart

When I go shopping I always eye the groceries of my neighbor before or after me. Sometimes it is scary to look at the content of some of the shopping wagons. There is bread, potato chips, pretzels, chocolate bars, cookies, and the whole line-up that is really rich in sugar. Manufacturers mix sugar into healthy yoghurt, so it becomes unhealthy. Honey or maple syrup makes granola unhealthy. Jams contain 50% sugar, and on and on it goes. It is sobering to see how illness and disability is for sale, one shopping at a time. The reassuring truth is that you have choices!

MIND Diet Helps Prevent Alzheimer’s

Researchers at the Rush University developed the MIND diet that helps prevent Alzheimer’s disease. MIND stands for “Mediterranean-DASH Intervention for Neurodegenerative Delay” and is a hybrid between the Mediterranean diet and the DASH diet that had been developed to help control high blood pressure.

The MIND diet was a prospective study where 923 people aged 58 to 98 years participated. Researchers followed these people for 4.5 years. Three groups of diets were tested: Mediterranean diet, DASH diet and MIND diet.

The adherence to the diet was measured: those who stuck to the diet very closely, another section of participants that were less diligent, and finally one segment of people who did not take the entire thing too serious. With regard to the MIND diet the group with the highest adherence to the diet reduced the rate of Alzheimer’s by 53% compared to the lowest third. The second group still was able to reduce the rate of Alzheimer’s by 35%. The control diets were the DASH diet and the Mediterranean diet. The group that were strictly adhering to the DASH diet reduced Alzheimer’s by 39%, the group that were very conscientious in adhering to the Mediterranean diet reduced Alzheimer’s by 54%. The middle thirds of both control diets did not show any difference versus the lower thirds. The conclusion was that a strict Mediterranean diet has a very good Alzheimer prevention effect, as does a strict MIND diet. However, when patients do not adhere too well to a diet, the MIND diet is superior still yielding 35% of Alzheimer’s prevention after 4.5 years while the other diets when not adhered to that well showed no difference from being on a regular North American diet.

A brief summary of what Alzheimer’s disease is

A few years back the Pittsburgh PET scan was invented. It uses a Pittsburgh B compound radio-tracer to visualize the pathology in the Alzheimer’s brain. This research tool has made it possible to detect even the earliest Alzheimer’s disease (AD). The basic problem in Alzheimer’s patients is that there is brain atrophy, which can be more located in the front of the brain or in the back. There are two types of AD, the early onset AD (EOAD) and the late onset AD (LOAD). Using the Pittsburgh PET scan it was noted that the brain atrophy with EOAD is severe and located in the back of the brain; in contrast to that with LOAD, which is the more common form of AD the brain atrophy is located in the front of the brain. This imaging tool also allows quantitating the amyloid load; amyloid is a gooey substance that is produced by the inflammatory changes in the brain of Alzheimer’s patients (Ref.1). There are multiple possible causes why Alzheimer’s should develop in a person, but nothing conclusive has been determined. In February, 2015 I summarized a talk given by Dr. Smith at the 22nd Annual A4M Las Vegas Conference that also mentioned that there are many causes that can lead to Alzheimer’s disease.

The bottom line is that atrophy of the brain leads to loss of memory, personality loss and premature death.

The MIND diet

This diet was developed by Martha Clare Morris, Ph.D., a nutritional epidemiologist of Rush University in Chicago and her colleagues. The team observed that some foods were associated with a delay in the onset of Alzheimer’s disease while others contributed to an earlier onset of Alzheimer’s.

This is how they developed the term of the 10 “brain-healthy food groups” and the 5 unhealthy food groups.

Meet the 10 “brain-healthy food groups”: green leafy vegetables, other vegetables, berries, beans, nuts, poultry, olive oil, whole grains, fish and wine.

The 5 unhealthy food groups are: red meats, cheese, pastries and sweets, butter and stick margarine, fried or fast foods.

The 10 brain-healthy food groups are full of anti-oxidants, resveratrol and healthy fatty acids. They prevent hardening of the arteries and prevent the deposits of beta amyloid in the brain, which prevents Alzheimer’s disease. There is a bit of an issue about wine, which I have reviewed in this blog. You can replace wine with resveratrol extract as a supplement in order to get away from the toxic effect of alcohol, which is a cell poison. The choice is yours.

Why are the unhealthy foods unhealthy? There is too much trans fat in butter, cheese and margarine, which accelerate hardening of your arteries. The same problem exists with red meats, pastries and fried or fast foods. Sweets pose another problem: the sugar causes insulin levels in your blood to raise, which in turn causes inflammation in the support tissue of your brain, called glia cells, which secrete the beta amyloid that damages brain cells. At the same time sugar is metabolized by the liver into fat and triglycerides, which causes hardening of the arteries. The end result is earlier onset of Alzheimer’s.

According to Morris blueberries and strawberries, which are part of the brain-healthy food groups have been shown to be the most potent berries in terms of protecting against Alzheimer’s disease and preserving cognitive function.

With the late onset AD (=LOAD) that is the most common form of AD genetic factors hardly play a role. But what we eat will determine whether or not we get AD and we can postpone it significantly, if we eat mostly foods from the 10 brain-healthy food groups.

The MIND diet has been shown to be able to postpone the development of Alzheimer’s disease. It was developed based on the observation that some foods are good for us in terms of preserving cognitive function whereas others are bad. Knowing this it is advisable to eat mostly vegetables, poultry, fish, olive oil, nuts, beans, berries, whole grains, and some wine may be enjoyed as well. At the same time you need to avoid the bad foods mentioned above, like pastries, sweets, red meat, cheese and fast foods.

It is a small price to pay to keep your brain function until a ripe old age. The advantage with the MIND diet is that if you should occasionally deviate from the ideal, you still maintain a significant advantage over the DASH diet or the simple Mediterranean diet.
