About your Bowel Movements

This article is about your bowel movements. Dr. Michael Camilleri is a consultant and professor in the division of gastroenterology and hepatology at the Mayo Clinic in Minnesota. He mentioned that in the Victorian era it was thought that a person should have a bowel movement every day. But now gastroenterologists agree that three bowel movements per week are satisfactory. If you don’t have that spontaneously, you are said to suffer from constipation. On July 12 CNN published a review about the same topic.

But other people who consume a lot of fiber may have a bowel movement two or three times a day. This, too is considered normal. But in addition to the frequency of bowel movements the consistency of it is very important as well. “The stool form, appearance or consistency of the bowel movement is actually a much better criterion than the simple numbers to frequency,” Camilleri said.

Bristol stool chart

The Bristol Stool Chart classifies stools into seven groups. With this in mind, the healthiest types of stools are types three and four. This is where the stool is shaped like a sausage with cracks on the surface or snakelike and smooth.

Experts say that if you are excessively straining during an attempt to poop or if you feel like you have not totally emptied after a bowel movement, you have to make changes. Either you need to poop more often or you need to get a healthier stool quality.

Your body posture influences the ease of your bowel movements

If you put your feet on a stack of books, this raises your knees above your hips. In that position your pelvic floor muscles relax, which supports your bowel. The end result is that poop passes through more easily. This advice comes from Dr. Trisha Pasricha, a gastroenterologist at Massachusetts General Hospital and instructor of medicine at Harvard Medical School. She commented further: “We did not evolve to have our bowel movements sitting with our hips at 90 degrees on a chair, which is what we do now. We used to all have bowel movements squatting. Sitting at that sort of upright, 90-degree angle actually closes off the passageway.”

Proper food intake necessary for a good bowel movement

Start with a Mediterranean type diet, which consists of a lot of vegetables, salads, fruit, whole grains and nuts. According to the FDA the daily fiber load should be about 25 grams or more. Kiwis and prunes are useful as a dietary fiber source. On the other hand, you don’t want to overdo the fiber intake as it can cause abdominal bloating and loose stools. A high fat diet slows your digestive system down. But caffeine and caffein containing beverages stimulate your bowels. In addition, regular physical exercise gets your bowels moving.

Constipation

Constipation is an important symptom that prompts patients to see their physician. There seems to be a lot of confusion in the general public about this topic. There are enormous cultural differences. For instance, in Africa where the population eats on average a much larger amount of fiber, the bowel movements are much bulkier.

Sir Dr. Burkitt, the famous English surgeon, examined bowel movements (stools) of African tribes in comparison to his English countrymen and came to the conclusion that in the Western world we need to remedy our constipation problem and cancer of the colon problem by eating more fiber.

What we know now

He is still right: fiber is mainly treating constipation (not preventing cancer of the colon), but the chemicals that are also in the vegetables contain a multitude of natural anti-carcinogenic substances, which provide the powerful preventative action against colon cancer and many other cancers. Lycopene is one of these. Tomatoes and tomato products contain it.

Sir Dr. Burkitt’s observation that high bulk food (with vegetables and green leaves) prevents cancer is as valid today as it was in the early part of the 1900’s.

Chronic constipation

Chronic constipation cases that start insidiously, but then remain despite taking a high fiber diet, make the doctor think about other underlying causes such as hypothyroidism (= low thyroid function) or metabolic causes such as hypercalcemia and uremia from early kidney failure.

We find that elderly people become too inactive. This lowers the natural peristalsis of the gut. In combination with poor eating habits and chewing problems because of poor teeth it is often responsible for chronic constipation. Also, elderly people often are on multiple drugs, all of which have a weak “anticholinergic” side effect, which translates into suppressing peristalsis chemically resulting in constipation. Psychogenic factors and chronic depression as well as obsessive-compulsive behavior can also feed into constipation. It can lead to a hyper awareness of one’s own bowel pattern. This is unhealthy and requires counselling. But first the doctor must rule out any serious cause of chronic constipation.

Treatment for constipation (simple steps that help)

As mentioned above, the patient has to modify the food intake to include as much fresh and steamed vegetables as possible. This will lead to bulk in the colon and make the stool softer thus allowing it to pass through the rectum and anal canal easier. The other advantage of this simple step is that the bulkiness of the stool triggers the normal peristalsis movement of the colon moving the contents towards and through the rectum. Decreasing the passage time in the colon allows less water absorption, keeping the stool soft and pliable until it leaves through the anal canal. This is easily achieved by a diet rich in fruits and vegetables. Cereals containing bran like slow rolled oats (not instant oats) are also useful when taken with breakfast. Cut down on meat and fat consumption.

Enema with lukewarm water

A simple tool for chronic constipation is an enema with lukewarm water (1500 ml or 50 fl.oz.) in a Faultless enema bag without any additives in it. The person who gets the enema needs to lay on the left side. This way the water can flow in easily into the colon. When entering the nozzle into the anal canal, do not force it. Instead use a bit of Vaseline ointment to facilitate entrance. The response usually comes within 5 to 10 minutes following the enema. It works by dilatation of the bowel wall, which leads to a reflex bowel contraction. Although it would be safe to take this every day, usually you should not have to take it more than two or three times per week even in chronic constipation.

Extra fiber (bulking agents)

Next, if this is not tolerated or does not appeal to the patient, I would recommend a bulking agent such as psyllium (brand names: Prodiem Plain, Metamucil, Novo-Mucilax) and bran (Brand names: Kellogg’s All Bran and Post’s bran flakes). These are mild laxatives, which are safe to take every day and which will not make the patient hypokalemic.

Osmotic agents

Osmotic agent: One or two tablespoons of sorbitol as a 70% solution is a hyperosmotic solution, which stays in the gut and draws water by osmosis into the colon. The bulking effect creates peristalsis and the water retention makes the stool softer. It takes often 1 or two days to get the full effect. There can be transient abdominal cramps until the stool is passed. One variation of this theme is to give sorbitol in a mix with other osmotic agents as a micro-enema in the form of Microlax (from Pharmacia and Upjohn).

The emergency break (laxative)

The “emergency break”: Occasionally all of the above is simply too weak. The constipated person normally is controlled with the above measures, simply could not go to the bathroom for several days. Bisacodyl (brand name: Dulcolax) and sennosides (brand names: Ex-lax Sugar-Coated Pills and Senokot products) can help you unload any held back stool on a one-time only basis. Increase your vegetable and fruit intake, cut down on refined foods and fatty foods, which are devoid of fiber.

Because of its role in nutrient absorption and body detoxification the digestive system plays a direct role in overall health and wellness. When the colon is unclean, waste and toxins can become trapped. This places an extra strain on the detoxification organs (kidneys, liver) and weakens the immune system causing illness. Being A Morning Person Can Prevent Depression

A British study concluded that being a morning person can prevent depression. It was reviewed also in CNN. The study used sleep data from 85,000 UK participants in the Great Britain Biobank Study. They wore wrist activity monitors that provided sleep data. Researchers compared the sleep data with the self-reported mood changes. They found that if the sleep pattern is misaligned with the circadian rhythm, those who are night owls are at a higher risk of developing mood disorders. The controls were those who were morning persons. They were not affected by the misalignment effect.

Normal sleep pattern

Natural sleep habit or the circadian rhythm starts between 10 PM and 11 PM and continues for 7 to 8 hours. Your hormones are replenished during your sleep. This helps your body’s hormones and the immune system to restore itself overnight.

“The health problems associated with being a night owl are likely a result of being a night owl living in a morning person’s world, which leads to disruption in their body’s circadian rhythms”. This is what sleep specialist Kristen Knutson said. She is an associate professor of neurology and preventive medicine at Northwestern University Feinberg School of Medicine.

Reclaim your natural sleep habit

The key is to learn to live within your circadian rhythm pattern. Caffeine is a powerful stimulant and will stop you from falling asleep. For this reason, it is best to avoid caffeine-containing beverages. If you cannot live without your favourite cup of java, switch to the decaffeinated version. Stop using LED lights (from TV, computers, tablets or smartphones) 2 hours prior to your bedtime. Use dark curtains and enjoy a comfortable bed. Maybe read that thriller, earlier in the day instead of making it your bedtime story. And do yourself a favour: you do not need the eleven o’clock news on TV!  They will probably stop you from falling asleep. Go to sleep between 10PM and 11PM.

Children can have problems with sleep disturbances and depression

Another study published March 22, 2021 in the Jama Network showed that depression had an association with sleep disturbances in youth and children.

A meta-analysis of 16 publications looked at depression and disturbed sleep. It showed that depression was 1.5-fold higher in sleep disturbed youths/children compared to controls with a normal sleep pattern.

Other studies re. being a morning person can prevent depression

Another study with Dr. Knutson as the lead author appeared in 2018. The authors found that various health conditions, mood disorders and mortality were on the increase the more the sleep rhythm deviated from the circadian rhythm. Morning persons were protected from this effect. But night shift workers and night owls were at a higher risk of disease. Specifically, they found the following associations for evening types.

  • Evening types compared to morning types had a 1.94-fold risk of psychological disorders
  • The risk of diabetes was 1.30-fold for evening types
  • Neurological disorders had a 1.25-fold risk in evening types
  • Gastrointestinal/abdominal disorders occurred 1.23-fold more often in evening types
  • respiratory disorders were 1.22-fold more common in evening types
  • Evening types had a 10% increased risk of all-cause mortality

The researchers concluded that externally imposed timing of work and social activities has potentially serious health consequences.

Circadian misalignment responsible for disease

Dr. Knutson also said: “Circadian misalignment could also lead to inadequate sleep duration and quality, which could also impair mood and exacerbate mood disorders.”

In other words, circadian misalignment to circadian rhythm problems. This can cause mood disturbances, but eventually lead to the diseases listed above.

Evidence of health risks in night shifts workers

The medical literature is full of examples that the health of night shift workers is significantly affected by circadian misalignment. Here are a few examples.

1.Here is a random selection to illustrate the health risks of night shifts workers. A study from 2015 examined the sleep patterns of 315 shift nurses and health care workers in Iranian teaching hospitals. They found that 83.2% suffered from poor sleep and half of them had moderate to excessive sleepiness when they were awake.

2.This South Korean study examined 244 male workers, aged 20 to 39 in a manufacturing plant. Researchers compared blood tests from daytime workers to blood tests from night shift workers. They also obtained inflammatory markers like the C-reactive protein and leukocyte counts. Night shift workers had significantly higher values. The investigators concluded that shift workers have increased inflammatory markers. This is a sign of a higher risk of developing cardiovascular disease in the future.

Higher mortality and higher cancer risk in nighttime workers

3. A Swedish study found that white-collar shift workers had a 2.6-fold higher mortality over a control group of daytime white-collar workers.

4. Another study compared night workers in the age group of 45 to 54 with daytime workers and found a 1.47-fold higher mortality rate in the night shift workers.

5.In a study from China 25,377 participants were part of a study that investigated cancer risk in males with more than 20 years of night shift work. They had a 2.03-fold increased risk to develop cancer compared to males working day shifts. Women with night shift work in this study showed no effect with regard to cancer development.

Healthy telomeres with healthy sleep pattern

It is true that you can suffer multiple health problems, as all of your hormones depend on the resetting during your deepest sleep between 2AM and 4AM triggered by the nighttime melatonin response. Even your telomeres, the caps of chromosomes in every cell get shortened from too much stress and too little sleep.

In this 2018 study researchers compared 46 nurses who had worked in night shifts with 51 nurses working day shifts. Among the night shift workers breast cancer was common, but not among day workers.

Shortened telomeres, hypomethylation of BRC1 gene and p53 gene

In the night shift nurses from Milan there was hypomethylation of the breast cancer gene BRC1. There was also hypomethylation of the general cancer gene p53. At the same time significant telomere shortening occurred in night shift nurses who had worked night shifts for more than 15 years. This likely all worked together in causing night nurses to develop breast cancer more frequently.

Shortened telomeres mean a shortened life span. The reason for this is that people with shortened telomeres develop heart attacks, strokes and cancer. This is what shortens the life span. How do we avoid this risk? Go back to healthy sleep habits. As mentioned above it is best to start going to sleep between 10PM and 11PM and sleep for 7 to 8 hours. Night owls delay going to sleep by 2 to 3 hours.

Conclusion

A publication in Molecular Psychiatry demonstrated that evening person (night owls) are more likely to develop depression. This is in comparison to morning persons. As discussed, other researchers showed that evening persons also can develop diabetes and neurological disorders. In addition, respiratory disorders and gastrointestinal disorders are more common in night owls as well. When it comes to mortality, evening persons have a 10% increase of mortality over morning persons. There is a large body of literature regarding diseases of night shift workers. Night Shift work is perhaps the most extreme example of a circadian misalignment. It leads to poor sleep, inflammation in the body, increased cancer risk and higher mortality compared to day shift workers.

Prevent telomere shortening

Even the telomeres get shortened in night owls and night shift workers. We can prevent problems like these by going to bed in time and sleeping according to our circadian rhythm. If you had a poor night’s sleep, make up for it with the help of an afternoon nap. Do not sleep all afternoon though; half an hour or one hour will be enough. Even a short, restful nap after coming home from work can restore your feeling of wellness.

The Best Ways To Prevent Cancer

Cancer is the second leading cause of death, that’s why it is important to learn about the best ways to prevent cancer. Generally speaking you want to remove cancer-producing substances (carcinogens) from your diet. But diet is only part of your lifestyle that can contribute to cancer. I shall list some of the more important dietary factors below and briefly touch on important other factors.

Avoid burning your meat

When you use the BBQ, you should avoid burning your meat. Even though the marks of “charring” are considered desirable on meat from the grill, this is really burnt! As a matter of fact it is a lot better to use a slow cooker at low heat and simply cook your food longer. This way you don’t create carcinogens. Avoiding to burn your meat is particularly important for the red meats.

Sugar and an overabundance of starchy foods can cause cancer

You never thought that sugar and an overabundance of starchy foods could cause cancer, but they do. The reason is that the metabolism of cancer cells is using 10- to 12-times more sugar than the metabolism of normal cells. The worst thing a cancer patient can do is to over-consume sugar. Replace sugar by stevia, which is a harmless, plant-based sweetener and does not lead to an insulin reaction. Avoid all other sugar substitutes, as there are other heath problems with most of them.

Avoid phthalates

Those who have a craving for macaroni and cheese are out of luck. This food contains phthalates that are part of the ingredients of almost every sample of cheese powder used to manufacture macaroni and cheese. Phthalates can cause infertility and breast cancer. So you must definitely avoid macaroni and cheese, at least the stuff from the box. Prepare your own!

A high fat diet

What does a high fat diet do? It increases the risk for breast cancer. But it can also increase colorectal cancer risk. Limit your fat intake to about 10% of saturated fat. That is the recommendation of the FDA. Increase your consumption of fish and seafood. Only one proviso: predator fish like shark, marlin, tilefish, swordfish and grouper are high in mercury. But wild salmon, sardines and oysters are low in mercury. You can also enjoy shrimps and prawns.

Take high dose vitamin D3 supplements

High dose vitamin D3 supplements help you to avoid cancer. There are strong statistics showing that vitamin D3 is a powerful tool to lower your risk of developing cancer. Your family doctor should take a blood test called 25-hydroxy vitamin D level to make sure that you absorb enough vitamin D3. There are slow and fast absorbers and the only thing to know how well your gut absorbs vitamin D3 is in doing this blood test.

Also, curcumin (turmeric) 500 mg once per day is good for cancer prevention.

Take enough fiber

Make sure you take enough fiber, which does not only reduce colorectal cancer, but also many other cancers. When you eat plant-based food, you automatically get fiber in it. North Americans are not consuming enough fiber in their diet.

Avoid processed meat and too much red meat consumption

Processed meat and red meat cause cancer.

Beef, lamb and pork seem to contribute to causing cancer according to the WHO. Use common sense and eat fish, chicken and turkey. Reduce your beef consumption. My grandmother served beef as a Sunday dish.The rest of the week simple, plant-based foods appeared on the table. Ask your grandmother, what she used to cook. Or ask your mother what she ate as a child.

Eat moderate amounts of fruit and vegetables

The claim that fruit and vegetables would protect you from cancer is not as solid as researchers thought of in the past. Newer research has shown that a basic intake of fruit and vegetables is needed for nutrients, but consuming more than that will NOT protect you from cancer.

This link explains that eating more vegetables or fruit beyond a certain point will not do harm, but will not protect you further from cancer.

Drink green tea or black tea

If you like tea, drink green tea or black tea. Sweeten it with stevia, but not with sugar. Tea has been shown to have cancer prevention properties.

Avoid alcohol consumption to prevent cancer.

Coffee is a healthy drink and it has mild anti-cancer effects as well. It does not matter whether you drink it caffeinated or decaffeinated.

Other lifestyle issues

Quit smoking

If you are still smoking, quit smoking! Smoking is by and large the biggest risk for developing lung cancer, throat cancer, esophageal cancer and pancreatic cancer.

Watch your calorie intake

Eat smaller meals more often. This way the production of your digestive juices will consume some calories. In addition your taste buds are satisfied, so your hunger for food is more controlled. The end result is that you will not gain weight.

Prevent obesity and type 2 diabetes

This will help prevent obesity and type 2 diabetes, both of which are established risk factors to develop cancer. Here is a review that shows you, which cancer types are caused by obesity. With regard to diabetes, there is a strong association to developing liver cancer, pancreatic cancer and endometrial cancer (=uterine cancer). There is a lesser risk (only 1.2 to 1.5-fold) to develop cancer of the colon and rectum, breast cancer and bladder cancer.

Pollution

Poor air quality with pollution can also be a factor in causing cancer. Pollution does not stay local, but travels through the stratosphere around the globe. The result is that now 10 to 15% of lung cancer in the US occurs in patients who never smoked. This translates into 16,000 to 24,000 deaths annually of never-smokers in the US. In certain cities such as Beijing lung cancer rates have doubled in 9 years between 2002 and 2011. Lung cancer in non-smokers can be caused from exposure to radon, to second-hand tobacco smoke, and other indoor air pollutants.

Bioidentical hormone replacement

When males do not replace missing testosterone in andropause they are much more prone to develop prostate cancer. Similarly, when women are menopausal and do not get progesterone supplementation, they develop a higher amount of breast cancer due to estrogen dominance. It follows from this that bioidentical hormone replacement in menopause and andropause will help to prevent prostate cancer and breast cancer.

There is strong evidence that certain foods can cause cancer. Other foods including supplements like curcumin and vitamin D3 can help prevent cancer. Basically, you want to avoid all that is known to cause cancer and eat more of the healthy foods that do not cause cancer. This will help to decline your cancer risk. I suggest that in addition you should quit smoking, avoid pollution as much as possible, reduce excessive alcohol intake and watch your calorie intake. By doing this you prevent obesity and type 2 diabetes, and you will move into the low-risk cancer group. We all need to work on this on an ongoing basis.

Practical hints regarding the best ways to prevent cancer

The best ways to prevent cancer is to avoid processed red meat and all other processed foods. Eat more vegetables, fruit, wild salmon and other seafood. Don’t eat red meat more often than once a week and make it red meat from grass fed animals. In addition exercise regularly, get enough sleep and practice some form of relaxation (yoga, Tai Chi, self-hypnosis etc.). This lifestyle will not only prevent heart attacks and strokes, but also the majority of cancers.

The Best Foods For Your Heart

In the following I will describe 16 foods, which are the best foods for your heart. I will also comment as to why I believe they are best. This review is based on this article in “Medical News Today”.

But I have added many other comments to it.

Heart disease is still the number 1 killer. We need to change what we eat.

Vegetables

The regular intake of green leaf vegetables and cruciferous vegetables (broccoli, cauliflower and cabbage) has an association with the  preservation of cognitive function. In the Nurses’ Health Study starting in 1984 women were asked about their usual intake of a specified portion of food items, including 15 fruits and 28 vegetables. At a later date, between 1995 and 2001, researchers decided to ask the oldest participants (70 years and older) to participate in a cognitive function study. Two years later researchers repeated these tests.The main finding of the study is that women with the highest intake of green, leafy vegetables had the least decline in their cognitive function. The vegetable lovers, who consumed five serving of cruciferous vegetables, like broccoli, cabbage and cauliflower had less decline in their cognitive function. On the other hand the highest decline showed up in the group that averaged only 2 servings per week.

Asparagus

Asparagus is a source of fiber, folate, multiple vitamins and chromium. Chromium enhances the function of insulin to transport sugar into cells. Asparagus contains glutathione, a powerful antioxidant that neutralizes free radicals and carcinogens. It is said to help prevent lung cancers, larynx cancer, and bone, breast and colon cancers.

Berries

Berries like strawberries, raspberries, blackberries and blueberries are full of bioflavonoids. These are antioxidants, which prevent cardiovascular disease.

It is the anthocyanines, which prevent oxidation of LDL cholesterol. Berries also have bioflavonoids and reduce lipid formation in the blood. Berries contain fiber, folate, iron, calcium, vitamin A, and vitamin C.

Broccoli

This fiber rich vegetable helps to prevent hardening of the arteries. Broccoli and kale likely have preventative effects against colon cancer.

Chickpeas, beans, peas and lentils

Legumes or pulses are a great way to consume plant-derived protein. People who are on vegan diets should be eating them for a protein source. They also contain lots of fiber, vitamins and minerals. We know that they lower cholesterol, which prevents heart disease. Other healthy nutrients they contain are bioflavonoids, which have antioxidant properties and help prevent cardiovascular disease.

Dark chocolate

This is a food rich in antioxidants. Dark chocolate is chocolate with more than 70% cocoa content. Please note: “milk chocolate” is nothing better than candy and devoid of any health benefits. Dark chocolate increases the protective HDL cholesterol and prevents oxidation of LDL cholesterol. It is said to prevent atherosclerosis (hardening of the arteries). Don’t exceed eating 1 to 2 oz. per day, as chocolate has some sugar in it and the fat content would be detrimental with higher consumption.

Chia seeds and flaxseeds

Chia seeds and flaxseeds are rich in omega-3 fatty acids in the form of α-linolenic acid (ALA). ALA reduces LDL cholesterol and it dissolves plaque in the arteries. ALA also reduces blood pressure to a certain degree. All of this helps reduce cardiovascular disease (heart attacks and strokes). Note that flaxseeds must be ground to powder to release the nutrients from its tough shell. Both chia and flaxseeds can be used as an egg replacement in vegan cooking.

Fish high in omega-3s

Omega-3 fatty acids are abundantly present in fish. It has plaque-reducing properties and also reduces the risk for abnormal heart beats. Overall this means less cardiovascular disease. The American Heart Association recommends a 3.5 oz. serving of fatty fish (salmon, mackerel, lake trout, herring or sardines) twice per week.

Coffee

A Brazilian study from 2018 showed that drinking 3 cups of coffee a day would stop coronary artery calcification. Many other studies have shown reduced mortality from heart attacks and strokes with increased coffee intake.

Green tea

Green tea is known to reduce blood pressure slightly and to reduce cholesterol. Both effects are beneficial for the cardiovascular system (prevents heart attacks and strokes). Green tea also prevents many cancers. Whatever we know about coffee consumption seems to also be true for green tea consumption.

Nuts

Nuts contain healthy fatty acids (omega-3). But they also contain fiber, protein, minerals, vitamins and antioxidants. Use them in desserts, in salads and as a quick food on the go. They are definitely healthier than protein bars.

Liver

Liver is one of the nutrient rich foods. It is rich in iron, phosphorus, vitamin A, folate, vitamin B12 and biotin.

Oatmeal

Oatmeal is rich in soluble fiber. It has been shown to lower LDL cholesterol and total cholesterol. This is important for prevention of heart disease.

Red wine (may be)

The bioflavonoids of red grapes have been the subject of much research. There is a dose-response curve showing a protective effect with regard to heart attacks and strokes with the consumption of  1 to 3 glasses of red wine per day. But unfortunately there is also a dose-response curve with respect to alcohol consumption and cancer causation. Personally, I take resveratrol from the health food store, 500 mg daily and consume white wine or red wine very rarely.

Tomatoes

There are a number of beneficial phytochemicals in tomatoes. Carotenoids like lycopene, lutein, zeaxanthine and beta-carotenes are helping to prevent prostate cancer and colon cancer. Potassium and folate are cardioprotective.

The Benefits Of Drinking Green Tea

Chances are you heard about the benefits of drinking green tea to reduce heart disease. But the polyphenols of green tea also prevent cancer, depression and cognitive decline.

Green tea lowers cardiovascular mortality

A study in the Chianti region of Tuscany, Italy consisted of 807 men and women aged 65 years and older. They had urine tests for metabolites of green tea in the beginning of the study. The study went on for 12 years. 274 participants died, which was 34% of the total study group. Tests measured the total urinary polyphenol (TUP) concentration in both groups, the survivors and the participants who died. Those participants whose TUP was in the highest tertile of the TUP values had the lowest all-cause mortality. And those participants whose TUP was in the lowest tertile had the highest all-cause mortality. 

High blood pressure and stroke

A Taiwanese study from 2004 examined the blood pressure of 1507 subjects. There were 711 men and 796 women with a recent diagnosis of  high blood pressure. The investigators looked at tea consumption (green tea and oolong tea) and blood pressure reduction. They found that those who drank 120 to 599 mL/day of green tea or oolong tea decreased their blood pressure by 46%. Those who drank more than 600 mL/day of green tea or oolong tea lowered their blood pressure by 65%.

Another meta-study involving 194,965 people and 4378 strokes found that there was a reduction of strokes with increasing tea consumption. Those who drank 3 or 4 cups of green tea or black tea per day were the experimental group. Researchers compared them  to the control group that drank less than one cup of tea per day. The experimental group had a 21% lower risk of getting a stroke than the control group.

Cancer prevention more with black tea than with green tea

A meta-analysis of 18 studies from China showed that green tea and black tea consumption was beneficial for prevention of cardiovascular disease and for cancer prevention. The highest consumption of green tea reduced cardiovascular mortality by 33%. The highest black tea consumption lowered mortality by 12%. Cancer mortality turned out to be different. Green tea did not produce a reduction in mortality, but black tea lowered it by 21%.

Another study, this one from Shanghai, China, points out that there are 15 polyphenols in green tea that likely work on different target areas of the body. They target 200 different genes in humans affecting diabetes, cancer, cardiovascular disease, neurodegenerative disease, muscular disease and inflammation.

Other cancers like prostate and breast cancer benefit from green tea

This study shows evidence that habitual green tea drinkers do prevent prostate cancer and breast cancer to a certain extent.

Another study investigates the effect of tea on health. One of the studies from the same authors have shown that the oxidative damage to cells from cigarette smoking can come out in a urine test. Those smokers who drank 4 cups of tea per day had a 31% decrease in their urinary biomarkers to indicate DNA damage. As DNA damage is often the first step in cancer development, these findings are important to note.

Cognitive effects of green tea consumption in dementia patients

A 2017 review of dietary supplements to improve cognitive impairment pointed out several supplements that will improve cognition. Green tea is one of them, but omega-3 fatty acids (EPA), and docosahexaenoic acid (DHA) are also useful supplements to restore neuronal functioning.

A 2017 study from Basel, Switzerland found that green tea improved memory, reduced anxiety and activated the working memory, which could be made visible on functional MRI scans. These researchers also pointed out that green tea has this effect as a whole, you cannot attribute it to a single constituent. Separate tests of  caffeine or L-Theanine  showed that the beneficial effect was smaller than when green tea as a whole was tested.

2016 study on severe Alzheimer’s patients

This 2016 Iranian study looked at 30 patients with severe Alzheimer’s disease. A baseline assessment was first, and 2 months after taking 4 green tea pills daily another assessment followed. A blood test measured the oxidative stress before and after, so was the antioxidant level from the green tea. The oxidative stress test showed an improvement during the study. The antioxidant level in the blood was higher than before the start of the study. The cognitive function test improved only slightly.

A 2018 study from Singapore looked at the effect of drinking tea (black tea or green tea) regarding symptoms of anxiety or depression. In a group of 614 subjects who were elderly individuals, aged 60 years and above; 59% consumed tea for longer than 15 years. These tea-drinking people were significantly less depressed and significantly less anxious when they underwent psychological tests, compared to non-tea drinkers.

There is a 2018 study from South Korea that looked at the link of beverages from 15 studies with depression. 347,691 participants were part  of  these 15 studies, and 20,572 cases of depression developed. Comparing a high consumption of tea or coffee to low consumption, the following statistics were the result: coffee consumption reduced depression by 27%, green tea consumption by 29%.

Bioflavonoids are powerful antioxidants. But there are many more substances in green tea and coffee that are all beneficial for our health. One of the studies mentioned identified 15 polyphenols in green tea. But another study said that trying to identify one of the components as more active than the others would be a waste of time. They measured some of the factors by themselves and found that the overall effect was much smaller than green tea as a whole.

Other research has shown that the components of green tea activate several genes. This includes anti-inflammatory effects, prevention of heart attacks and strokes, lowering of blood pressure, anti-cancer effects, improvements of cognitive function, as well as improvement of depression and anxiety. As you can see the effect of green tea is diversified. Don’t hesitate to consume another cup of tea!

3 Cups Of Coffee Stop Clogged Arteries

There is a new coffee study that says 3 cups of coffee stop clogged arteries. The study appeared in the Journal of the American Heart Association on March 24, 2018. It was part of the cross sectional Brazilian Longitudinal Study of Adult Health. This study took place from August 2008 to December 2010. It followed 15,105 civil servants, aged 35 to 74 years. They were living in 6 Brazilian cities (Belo Horizonte, Porto Alegre, Rio de Janeiro, Salvador, São Paulo, and Vitoria). The final study sample comprised 4426 individuals who underwent CAC measurements.

CAC score

CAC stands for “coronary artery calcium”. Other tests in the past have shown that CAC is a very sensitive test that quantitates the calcification status of coronary arteries. The CAC score is also known as Agatston score. It also predicts how likely it is that a person with a high CAC count will develop a heart attack down the road. A score of 100 or less is considered to be a low risk for developing a heart attack later in life. If the score is between 100 and 400, it is a moderate risk. A score of more than 400 ranks as a high risk to develop a heart attack.

Study design

Upon entering the study the participants had an extensive entrance exam and interview. Part of this was a nutritional and lifestyle questionnaire. This told the examiners whether the patients were smokers or not. Their age ranged between 35 and 74 years. Participants took a CAC test. 4426 individuals who underwent CAC measurements became part of the study. The participants’ blood pressure and body weight were additional measurements.

Results of the study

There was a negative correlation between the amount of coffee consumed on a daily basis and the CAC score. Specifically, the following observation emerged regarding daily coffee consumption and the risk of a higher than 100 CAC score.

  • Less than 1 cup per day: odds ratio 0.85
  • 1 to 3 cups per day: odds ratio 0.73
  • More than 3 cups per day: odds ratio 0.33

These results show clearly that there is an inverse relationship between coffee consumption and risk of developing coronary artery hardening. Those who consume 3 or more cups of coffee have a lower coronary artery disease risk.

The risk to develop coronary artery calcification in coffee drinkers (3 or more cups) is 67% lower. The comparison group are non-coffee drinkers.

Discussion of the results

The research paper from Brazil compares their results with many other research facilities and concludes that others had very similar conclusions. For instance, a 2014 publication pointed out that consuming more than 5 cups of coffee per day had no association with heart disease and did not cause any increased mortality.

The 2015–2020 Dietary Guidelines for Americans recommends consuming 3 to 5 cups of coffee per day, which reduces the risk of getting diabetes type 2 or cardiovascular disease.

Korean study using CAC scores

One other relevant Korean study that involved 25,138 participants is also of note. The participants consumed 3 to 5 cups of coffee per day. They also underwent CAC tests. The coffee drinkers had a 41% lesser prevalence of CAC score in comparison to non-coffee consuming controls. The authors of the present study said that the Korean study was comparable to their own findings.

Comparison of study to previous literature

Mortality data and coffee consumption

In a large Harvard study that I discussed on Nov. 18, 2017 the end point was mortality data. There too, was a dose response curve. With 3 to 5 cups of decaffeinated or regular coffee there was a reduction of death rates by 15%. Less coffee consumption produced less reduction of mortality.

Stroke data and coffee consumption

In another review on March 4, 2013 I reviewed a 10-yer study that looked at the effect of 1 to 2 cups of coffee per day in 32,600 women. There was a 25-32% reduction of strokes compared to non-coffee or tea consuming controls. I also reviewed a Finnish study that consisted of 29,133 smokers (smoking 5 cigarettes a day) aged 50 to 69 who were stroke free when entering the study. It ended in 1993 and the study had lasted for at least 5 years. Drinking two cups of black tea or 8 cups of coffee reduced the stroke risk by 21 to 23%.

Stroke mortality and coffee consumption

In the Annals of Internal Medicine (June 17, 2008) researchers have found that coffee consumption can be beneficial to reduce stroke mortality. A study using the data from 41,736 male and 86,214 female subjects is quite informative. Researchers followed these people over 18 years in the male group and over 24 years in the female group. The risk of all-cause mortality decreased significantly with increasing coffee consumption in the male as well as in the female group. Men had a relative risk reduction of 20% in comparison to those with the lowest level of coffee consumption. Women with intermediate to high consumption of coffee had a relative risk reduction of 10% to 30 % when compared to the group that drank less than 1 cup of coffee per day. The end point here was risk of death from stroke.

Conclusion

Coffee has a number of anti-oxidant bioflavonoids that help increase survival. The main study reviewed measured the effects of coffee consumption by calcium scores of the coronary arteries. This is a specific way to look at the relative risk to come down with a heart attack in the future. Other ways to measure the beneficial effect of coffee are mortality as the end point, stroke or stroke mortality. I have reviewed some studies that used all of these end points. It is interesting to observe that consumption of 3 to 5 cups of coffee has such a beneficial effect. It does not matter whether it is regular or decaffeinated coffee. Both have the identical effect. This has been shown by a recent Harvard study.

Coffee and tea make you live longer

Coffee keeps the arteries open for longer, if we consume 3 to 5 cups of coffee per day. This allows our brain to think longer and allows our heart to beat for many more years. It is time to stop the research and to apply what was found: 3 to 5 cups of coffee per day. If tea is what you prefer, you are in luck. Everything I said about coffee also applies for green and black tea. There is one word of caution. Using copious amounts of sugar in your coffee or tea will most likely cancel some or all of the benefits. Cream and sugar-laced Frappuccino or Chai Lattes are calorie bombs and not conducive to good health. I use stevia instead of sugar to sweeten my coffee.

You May Want To Cut Down Coffee Consumption

Many people drink too much coffee, so you may want to cut down coffee consumption. With all the good news about the health benefits when drinking coffee, some people went too far. They have overdone what was supposed to be good for them. Recently a study came out that tells you how to cut down coffee consumption.

But first I like to review the issue whether to drink caffeinated or decaf coffee. Next I will tell you how you can switch to decaf coffee.

Caffeinated and decaffeinated coffee have the same health benefits

  1. Recently a large study showed that coffee, caffeinated or not, has a connection with lower overall mortality.
  2. Coffee has long been a subject of heated discussions. Some praise it, and others condemn it. There are multiple past studies; some showed health benefits, some did not. This is why the Department of Nutrition, Harvard School of Public Health in Boston, MA. did a larger study. The purpose was to re-examine the health benefits for both caffeinated and decaffeinated coffee.

Mortality data regarding people who drank decaf coffee or regular coffee

Researchers assessed mortality among 74,890 women in the Nurses’ Health Study (NHS). Another 93,054 women in the NHS 2 study became part of this. And 40,557 men in the Health Professionals Follow-up Study were also part in this large study. The medium follow-up for all of these three groups was 22.5 years. 19,524 women and 12,432 men died during that time period. Ming Ding is a doctoral student at the Harvard School of Public Health department of nutrition. She was the lead author of this study. She pointed out that in the past there were confounding problems. Many studies had shown that both caffeinated and decaffeinated coffee consumption lowered the risk of cardiovascular disease. But the results in many studies were blurred. Studies often did not distinguish between smokers and non-smokers. This meant that the cardiovascular risk from smoking wiped out a beneficial effect from coffee drinking.

Confounding and other factors

Ding’s studies took this into account and also other confounding factors like how much sugary soda pop people were drinking and whether or not they were eating well. In addition they normalized for other factors that could interfere like drinking alcohol and eating red meat. Without normalizing for the factors mentioned above the study results were as follows. Study participants who had less than a cup of coffee and three cups a day had a 5% to 9% lower risk of dying than those who drank no coffee. Those who drank more than three cups a day did not see any benefit.

Dose response curve for regular and decaf coffee

After eliminating all the confounding factors researchers compared the various groups again, and the following linear dose-response curve emerged:

  • Less than 1 cup of coffee per day: 6% lower death rates than non-coffee drinkers.
  • 1 cup to 3 cups of coffee per day: 8% lower death rates.
  • 3 to 5 cups of coffee per day: 15% lower death rates.
  • More than 5 cups of coffee per day: 12% lower death rates.

Coffee consumption reduces diabetes and heart disease

Ming’s study connected with another research paper that had shown that coffee drinkers have a lower risk of developing type 2 diabetes and also less heart disease. She found that both, caffeinated and decaffeinated coffee, reduced the risk of getting diabetes later in life. When asked about what would be responsible for the reduced death rates with coffee consumption, she explained: “There are at least two known chemicals in coffee, namely lignans and chlorogenic acid that could reduce inflammation and help control blood sugar, both of which could help reduce the risk of heart disease”. You may want to cut down coffee consumption because you know decaf coffee does the same as regular coffee.

Other details about the caffeinated/decaf coffee study

Although there seems to be a linear response up to 5 cups of coffee consumption, above 5 cups this linear relationship disappeared. It was not explained whether there was a saturation point, whether there was yet another hidden confounding factor or whether there were detrimental effects on the adrenal glands with too much caffeinated coffee consumption.

Another finding was that it did not matter whether the coffee was regular (caffeinated) coffee or decaffeinated coffee. The results were identical.

Many other studies did not have the large numbers to show whether or not decaffeinated coffee was as effective in preventing heart disease as regular coffee.

Suicide rates and coffee consumption

There was another peculiar finding: suicides were down by 20% to 36%, if a person drank at least one cup of coffee per day. If a person consumed less than 1 cup of coffee per day the suicide rate was 36% higher than the control group with no coffee consumption. This is a rather peculiar finding, particularly for the consumption of less than 1 cup of coffee. Other studies also showed a decrease in suicide rates with coffee consumption.

Although previous studies had shown a reduction in liver and prostate cancer, after the removal of confounding factors this study did not show any effects on cancer causation or cancer death rates with coffee consumption.

Discussion

The Department of Nutrition, Harvard School of Public Health in Boston, MA has excelled in high quality nutritional studies for decades. This study is particularly important, because it is so large, giving it more statistical power. Secondly, the observation time of an average of 22.5 years is longer than most coffee studies in the past. Add to this the removal of the “noise” (called confounding factors) that interfered with the objective of the study, and you end up with a very meaningful result.

Clear results after confounding factors were removed

The important findings were that both caffeinated and decaffeinated coffee have the same effect of saving and extending lives. Perhaps you want to drink not more than 5 cups of coffee per day. That lowers your risk of premature death by 15%. It is most likely that it is the effect of lowering the rate of diabetes and heart attack rates that is responsible for the risk reduction. At least this was the opinion of the chief investigator. Cancer rates were not lowered by coffee consumption.

I sleep better when I drink decaffeinated coffee, so for me the notion that decaffeinated coffee and regular coffee have the same effect was important.

Revisit the statement: “you may want to cut down coffee consumption”

Now we know that there is no difference in benefits whether the coffee is caffeinated or not. Those of you who consume 3 to 5 cups of decaf coffee already enjoy a 15% reduction in risk of cardiovascular disease.

Those of you who take the same amount of regular coffee may get into a caffeine dependency problem. Because every time the caffeine stimulation wears off, you yearn for yet another cup of coffee. You need your fix, and this becomes a dependency problem. You have conditioned your body to that regular dose of caffeine, even though it is the bioflavonoids that are reducing mortality while caffeine is neutral.

My experience of coffee withdrawal

When I came across Ding’s research findings I was glad that now there was clarification about whether decaf coffee was as good as regular coffee. The next step for me was to cut out regular coffee and replace it by decaf coffee. Formerly I had been drinking 5 mugs of coffee daily (translated into 500 mg of caffeine daily). When I decided to quit this habit, I figured I should do it cold turkey from one day to the next. To my surprise this was a much bigger deal than I had thought.

Withdrawal symptoms

I craved the next cup of coffee, and I drank a decaf coffee. It did not help: Still, there was this craving for regular coffee! Yawning, restlessness and tiredness were symptoms that followed me all day long. Then there was irritability, a mild headache and almost flu-like symptoms. Eventually I went to sleep and woke up one hour later feeling a bit more energetic. But two hours later I had to lay down again. I was feeling that bushed. The following few days went better. There was more energy. But I still liked a noonday nap of about 1 hour.

Benefits of getting off regular coffee

This was not like me! Normally I have lots of energy and I don’t need naps. It took me 1-½ weeks to get over my 5-cup a day coffee withdrawal. But it was 100% worth it! Since then my energy is back to normal. I don’t have to chase coffee houses on a trip or ensure there is always a cup of regular coffee available for me at home (work does not apply, because I am retired). If I want I can replace my beloved coffee with another fluid. I love lemon juice sweetened with stevia instead of my decaf coffee. It is liberating that I no longer depend on the caffeine. But I still like the flavor of decaf coffee, and there is something enjoyable about the fragrance of freshly brewed coffee. And so I drink 3 to 4 cups of decaf coffee a day.

How to cut down coffee consumption

Here is a 2016 study from the Johns Hopkins University where 34 patients on 600 mg of caffeine per day received a 1-hour lecture about coffee withdrawal followed by a 6-week diary of their coffee consumption. They were asked to reduce their caffeine consumption down to 50 mg by week 6 of the coffee elimination program. Tests followed with salivary caffeine levels 6, 12 and 26 weeks after coffee cessation. There was also a 1-year follow-up telephone conversation. The results were that there was good compliance. Saliva caffeine levels verified this. The diaries over the first 6 weeks showed that the participants had gradually eliminated caffeine consumption. Perhaps this was a more humane way than my “cold-turkey” approach.

Conclusion

Many people are sensitive to too much caffeine consumption in coffee and other caffeinated beverages. But since the Harvard study that I mentioned above there is no need to overdose coffee or tea consumption. Decaf coffee has the same effect on lowering death rates by 15%, as does regular coffee. It pays to avoid caffeine, as you will avoid caffeine dependency. Drink decaf coffee instead!

I also discussed that withdrawal from regular coffee can be done more gently over a 6 week period. I did it from one day to the next and had a 1-½ week long withdrawal reaction. Do it slower or faster, whatever works best for you. The end result will be the same. Then enjoy it that you no longer depend on caffeine!

Parkinson’s Disease May Be Stopped

Parkinson’s disease is common in the US; new research shows that the use of an old anti-depression medication can stopParkinson’s disease The use of nortriptyline, a 50-year old antidepressant has shown to normalize a nerve cell protein. In rats nortriptyline dissolved toxic alpha-synuclein clusters in brain cells. These toxic protein clusters seem to be happening in the brain of Parkinson’s disease patients also. It is the protein by the name of alpha-synuclein that research first found in rats to cause the toxic protein clusters in nerve cells of the substantia nigra, a part of the brain stem.

But nortriptyline was able to normalize the concentration of the protein. In preliminary studies in humans the investigators found that there was a significant improvement of Parkinson’s disease with the use of nortriptyline.

Placebo controlled trial with nortriptyline

Now a research team from Michigan State University in Grand Rapids conducted a larger clinical placebo-controlled trial. The lead researcher Collier of the study group found that Parkinson’s patients who received treatment for depression with the tricyclic agent nortriptyline needed less dopamine, the main drug used to treat Parkinson’s disease. This indicated to the researchers that nortriptyline was preserving brain cells that were still making their own dopamine. In rat experiments they could show that it was the dissolving of toxic alpha-synuclein proteins by nortriptyline that was the key to therapeutic success.

Lisa Lapidus, a co-worker on the Michigan State University research team summed up their research: “What we’ve essentially shown is that we are dealing with a drug that the FDA approved already 50 years ago. Patients tolerate the medication relatively well. This could be a much simpler approach to treating the disease itself, not just the symptoms.”

Parkinson’s disease may be stopped also by old diabetes drug

Thomas Foltynie found that the diabetes drug exenatide helps patients with Parkinson’s disease. Dr. Foltynie is a professor of neurology at the University College London and co-author of the study.

Exenatide is an injection drug. When preliminary studies showed that this drug was effective in helping Parkinson’s disease patients lose their problems with walking and balance, a formal study followed.

Professor Foltynie designed a study where 60 people with Parkinson’s disease either got injections of exenatide or placebo injections. Patient exams followed regarding their musculoskeletal system and balance at baseline and every 12 weeks. A score system of 132 points assessed their Parkinson’s disease. After 48 weeks those who had been taking exenatide had a gain of 1 point on that scale while the placebo group dropped 3 points. After 48 weeks the drug administration (exenatide) finished. But after another 12 weeks another scoring and assessment of the Parkinson’s disease symptoms took place. The experimental group on exenatide scored 3.5 points higher than the placebo group. This suggests that exenatide is helping to treat the cause of Parkinson’s disease, not just the symptoms.

Parkinson’s disease may also stop through the use of caffeine

Parkinson’s disease was in the news again because of another study that involved breaking up misfolded alpha-synuclein through caffeine.

Misfolded alpha synuclein forms clumps inside dopamine producing cells in the substantia nigra of the brain stem. Misfolded alpha synuclein acts like a toxin to the dopamine producing cells and eventually these cells die off. This is the brain region that is responsible for making muscle movements smooth and stabilizes balance. The cells that have misfolded alpha synuclein clumps in them also go under the name of “Lewy bodies”.

Dr. Jeremy Lee from the University of Saskatchewan (Saskatoon, Saskatchewan, Canada) has isolated two compounds from coffee. They are called C8-6-I and C8-6-N. They can bind to alpha-synuclein and prevent clumping, which stops the toxic effects on dopamine producing nerve cells. Like with nortriptyline the caffeine effect is a curative approach to Parkinson’s disease.

There is a new therapeutic approach to Parkinson’s disease. Researchers have detected a protein called alpha-synuclein to cause toxic protein clusters in nerve cells of the substantia nigra, a part of the brain stem. When these cells die from the accumulation of these misfolded proteins, patients come down with Parkinson’s disease. But three different methods of treatment can improve Parkinson’s disease by dissolving the protein alpha-synuclein.

  1. Nortriptyline was able to normalize the concentration of the protein. In preliminary studies in humans the investigators found that there was a significant improvement of Parkinson’s disease with the use of nortriptyline.
  2. Exenatide, an injection drug for diabetes, has been described to help Parkinson’s patients get better.
  3. Caffeine can also dissolve misfolded alpha synuclein (two compounds from coffee called C8-6-I and C8-6-N). This helps patients with Parkinson’s disease to stabilize.

Getting Rid Of Sugar

Scientific papers describe how unhealthy added sugar is, so getting rid of sugar in your diet is the answer.

An article was published in CNN that describes a one-month sugar detox program.

Brooke Alpert, the co-author of the book “The Sugar Detox: Lose the Sugar, Lose the Weight — Look and Feel Great” describes the details of a sugar detox program.

 The first three days of sugar detox

You need to go cold turkey for three days to eliminate any sugar to break the addiction cycle. During this time you may experience anxiety, sugar craving and moodiness.

You must not consume any added sugars during the first 3 days, but also keep away from fruit. Avoid starchy vegetables. This includes corn, sweet potatoes, peas and butternut squash. You also need to avoid dairy, because of the milk sugar in it. In addition avoid grains and alcohol. “You’re basically eating protein, vegetables and healthy fats”, says Alpert. You would enjoy a breakfast with 3 eggs any style. Lunch contains 6 oz. of poultry, tofu or fish. Add a green salad. For dinner your helpings are bigger, but it would be similar to lunch, perhaps with added vegetables. Snacks can be 1 oz. of nuts or a few slices of bell peppers with hummus. Your beverages: unsweetened tea, black coffee or water.

People should avoid sweeteners according to Alpert. Dr. Robert Lustig, professor of Pediatrics and member of the Institute for Health Policy Studies at the University of California, San Francisco agrees. He said that artificial sweeteners cause you to store more fat. Dr. Lustig added: “You also end up overeating later on to compensate for the increased energy storage”.

Day four of sugar detox

On day 4 the patient can reintroduce an apple. It will now taste a lot sweeter, because in the past sugar suppressed the natural fruit sugar taste. Full fat, unsweetened cheese and yogurt are also part of the diet. Alpert added: “Fat, fiber and protein slow the absorption of sugar, so taking out fat from dairy will make you absorb sugar faster.” Higher sugar content vegetables like carrots, snow peas and high-fiber crackers can be part of the diet in the second half of the first week.  Those who like it can also add three glasses of red wine.

Second week of sugar detox 

Add a serving of berries that are full of antioxidants. Also add an extra serving of dairy. You can also eat more starchy vegetables like winter squash and yams.

Third week of sugar detox

Barley, quinoa and oatmeal can now be added back. Grapes and clementines are now also allowable. Those who like wine can now add another glass of red wine. One ounce of dark chocolate per day can also be added. “Week three should be quite livable,” Alpert said.

Week four of sugar detox

You now can have two starches per day, like bread and rice. In addition you can enjoy high-fiber crackers. You can have 5 glasses of red wine per week. You may want to eat a sandwich. Once you have completed 31 days no fruit is off-limits. You can allow yourself the occasional indulgences like an ice cream or a piece of cake at a birthday party. “Because the addictive behavior is gone, having ice cream once or twice will not send you back to square one. The whole purpose is to give people control and ownership and a place for these foods in our life,” Alpert said.

Weight loss with sugar detox

There can be a lot of weight loss with a sugar detox; depending on how much sugar the person was consuming before the detox started. Alpert recalled that of 80 subjects who tested the sugar detox people lost between 5 and 20 pounds within 31 days. Many people reported that their belly fat was going and they had to readjust their belts. There were other things participants noted: brighter eyes, fewer dark circles around the eyes, clearer skin, more energy and not as many mood swings.

Many people are not properly educated; they reduce fat intake, but add sugar and start gaining weight. By cutting out sugar and adding healthy fats in their diet, they now feel satisfied. The sugar craving has disappeared.

Side effects of sugar detox

You may feel grumpy and weak. This likely is due to low blood sugar and a piece of fruit will help you overcome this. In other cases it could be due to too little fluid intake and when you drink some water, this feeling will pass. Make sure you have supportive family and friends through the first three-day of sugar detox. “You need people around you to help you be successful,” Dr. Lustig said. “The whole family has to do it together.” Some irritability is expected due to sugar and caffeine withdrawal. This will pass and get better with every day. There are persons who should not go on this program: diabetics, people who take medicine to control blood sugar and extreme athletes. Pregnant women should also not follow a sugar detox program.

Why it is important to limit sugar intake

The American Heart Association (AHA) clearly spelled out what the limits are for added sugars:

For men: a daily dose of 9 teaspoons of sugar from all foods,

For women: a daily dose of 6 teaspoons of sugar from all foods.

When you start reading labels, which I started to do since 2001, you realize that it is very easy to exceed these daily limits. What does this do? It makes your pancreas produce more insulin than is good for you. You start switching your metabolism into the metabolic syndrome, you gain weight, and you get inflammation. This is what the AHA is concerned about. You start developing high blood pressure; get high LDL cholesterol, high triglycerides, and atheromatous plaques in your coronary and brain arteries. Eventually you come down with a heart attack or a stroke.

In the last 100 years sugar consumption per person has skyrocketed.

This has led to a disbalance of our diet and it is because of this that we are facing the obesity wave now. All I am saying is replace sugar with stevia extract, which to my knowledge does not have the undesirable side effects that other sweeteners do and eat a Mediterranean type diet, which we know is balanced.

Watch the maximum recommended sugar intake per day set by the American Heart Association and balance your meals, and you will do a lot better than on the Standard American diet. The Mediterranean diet is anti-inflammatory.

New Treatments For Premenstrual Syndrome

Dr. Pamela W. Smith gave a talk about new treatments for premenstrual syndrome (PMS). She presented this talk on Dec. 11 at the 24th Annual World Congress on Anti-Aging Medicine (Dec. 9-11, 2016) in Las Vegas that I attended. The original title was “New Treatment Modalities for PMS”.

Signs and symptoms of PMS

Dr. Smith discussed signs and symptoms of PMS first. She showed 9 slides where she listed all of the symptoms of PMS that commonly occur.

Symptoms are varied; they can mimic various psychological problems like anxiety, depression, panic attacks and more. But physical symptoms like abdominal bloating, acne, back aches, and asthmatic attacks are also common. There are a myriad of more symptoms of PMS: constipation, cramps, clumsiness, dizziness, drowsiness, decreased sex drive, facial swelling, forgetfulness, fatigue, headaches, a herpes-like outbreak, hot flashes, sensitivity to light and noise, insomnia, joint pains, mood swings, palpitations, restlessness, poor memory, sore throat, tearfulness, vomiting and weight gain.

What do we know about PMS?

PMS is due to a hormone dysfunction

There is no definitive test that would help in the diagnosis of PMS. But we do know that there is a hormone dysfunction that leads to a monthly recurrence of symptoms during the two weeks prior to the woman’s menstruation. When her period begins or shortly after all of these symptoms disappear.

PMS due to estrogen dominance

PMS is very common; 70to 90% of women have a certain degree of PMS. In 20 to 40% of women symptoms are severe. Many researchers have shown that there is a problem in the feedback loop between the pituitary gland and the ovaries. This leads to a decrease of progesterone production in the ovaries. The result is an overabundance of estrogen, which many hormone experts call estrogen dominance.

Several hormones need checking with PMS

But things can get complicated when other hormone changes occur. A woman may also turn hypothyroid. When she gets closer to menopause estrogen deficiency may also develop. Electrolyte disturbances can occur from high estrogen levels causing excessive aldosterone levels. This would lead to high sodium and low potassium blood levels. The end result may be an activation of the renin-angiotensin system, which could cause high blood pressure.

Neurotransmitters of the brain can be involved in PMS

Neurotransmitters are often disbalanced. When serotonin is low in the brain, depression can develop. Noradrenalin deficiency leads to a lack of focus, energy and memory.

Women with PMS often have hypoglycemia

Hypoglycemia is also common among women with PMS. This may be due to cravings for sweets and consumption of starchy foods. Women who crave sweets may also consume caffeine excessively. But caffeine increases prostaglandin production in the body. This leads to breast tenderness, abdominal cramping, arthritis and back pains.

PMS after partial hysterectomy

A partial hysterectomy can cause PMS in a woman who never before had symptoms of PMS. The current school of thought is that blood supply to the ovaries after a hysterectomy has decreased, and this could be the reason for the onset of PMS.

Birth control pill can cause PMS

The birth control pill can bring on PMS due to the progestin component in it.

Tubal ligation may cause PMS

Tubal ligations can also be a cause: 37% of women who had tubal ligations develop PMS. The reason is a change in hormone production. In these women estrogen is produced to a higher degree than progesterone is.

Lab tests for women with PMS

Although there is no single test that would be able to diagnoses PMS, a variety of abnormal tests are often abnormal in association with PMS. Frequently there is deficiency for vitamin A, B6, E, magnesium, potassium, zinc and trace minerals. Calcium can be too high or too low, but blood tests will reveal that.

Four PMS types

Dr. Smith said that PMS has been divided into 4 subcategories depending on the main symptoms.

  • Anxiety: PMS A
  • Carbohydrate craving: PMS C
  • Depression: PMS D
  • Hyperhydration: PMS H

PMS A is associated with estrogen excess and progesterone deficiency. There is a diminished stress response in the hypothalamus/pituitary/adrenals axis. Symptoms are mainly anxiety, irritability, insomnia and emotional lability.

PMS C is associated with sugar craving, hypoglycemia, headaches, heart palpitations and spontaneous sweating.

PMS D is leading to increased neurotransmitter degradation. Symptoms consist of depression, crying, despair, feeling hopeless, fatigue, low libido, apathy and insomnia.

PMS H is caused by increased aldosterone activity triggered by estrogen surplus in the late luteal phase. Symptoms are weight gain, swelling of hands and feet, a feeling of bloating, breast tenderness or engorged breasts. Women will find that their clothes simply fit tighter.

Migraine headaches in PMS

Some women with PMS are plagued by migraine headaches. It may have started in puberty or after taking birth control pills for contraception. Sometimes the onset is after a pregnancy, miscarriage or abortion. When PMS develops and a woman has migraines, they usually occur around the same time in her menstrual cycle. With pregnancy the migraines disappear in the last trimester when progesterone hormone production from the placenta is the highest.

Hormonally related headaches can occur for 4 main reasons.

  1. Estrogen and progesterone are on the rise around the time of ovulation
  2. When a woman has hypoglycemia (due to hyperinsulinemia)
  3. Estrogen levels are changing
  4. When there is estrogen dominance

Treatment for PMS

Dr. Smith explained in detail the various treatment modalities for PMS. Treatment has to be personalized according to what type of PMS the doctor thinks that the patient is suffering of.

Dietary factors

In the beginning it is important to pay attention to the diet. Studies have shown that PMS patients tend to eat too many carbs and too much refined sugar compared to patients without PMS. PMS patients also eat too many dairy products and too much sodium. In addition PMS patients are deficient in iron, manganese and zinc. A good start is a Mediterranean diet, which is at the same time anti-inflammatory.

The recommendations is to eat 6 small meals a day. Avoid caffeine and alcohol. They are antagonists to the B complex vitamins. By avoiding sugar, you also avoid fluid accumulation and magnesium deficiency. A low fat, high-complex carb diet helps reduce breast tenderness. Reducing fat and increasing fiber in the diet decreases estrogen levels in the blood. These steps help PMS symptoms.

Nutritional supplements

Magnesium, vitamin B6, A, D3, E, L-tryptophan, calcium, zinc, fish oil (EPA/DHA) and evening primrose oil are the main supplements recommended for PMS patients. Your healthcare provider can advise you what you should take and what dosage.

Exercise

Regular exercise has very beneficial effects on reducing many symptoms of PMS. Even as little as 8 weeks of exercise -as was done in this study- had a significant effect. Exercise elevates endorphin levels, improves blood sugar stability, decreases norepinephrine and epinephrine in the brain and helps to decrease estrogen levels. This will control blood sugar levels, reduce anxiety, reduce estrogen-related symptoms and increase satisfaction.

Thyroid medication

Many women with PMS have borderline hypothyroidism or are overtly hypothyroid. In these cases the patient should receive small amounts of thyroid hormones.

Progesterone

Most PMS patients persistently lack one hormone,  progesterone. The best test for this is a saliva hormone test, because this reflects the tissue levels. Blood levels test too low and are useless. Bioidentical progesterone cream is applied transdermally (through the skin) from day 14 to 25 of each menstrual cycle. Micronized progesterone pills are also bioidentical and can take the place of progesterone cream.

Botanicals

There are a number of home remedies, which are heavily promoted on the Internet. They may, however, not be as effective as advertised.

Estrogen balancing Black Cohosh

Black Cohosh is said to balance estrogen and is anti-spasmodic.

Progesterone raising Chasteberry

Chasteberry decreases LH and prolactin. It raises progesterone, acts as a diuretic and binds opiate receptors. This reduces PMS related aches and pains.

Herbal supplement St. John’s Wort

St. John’s Wort helps these symptoms: anxiety, depression, mood swings, feeling out of control and pain.

Anti-inflammatory Ginkgo biloba

Ginkgo biloba is a mild blood thinner. Women who are on blood thinners should not use it! It improves depressive symptoms and mood, has anti-inflammatory effects and helps with anxiety control.

Nutrient-rich saffron

Saffron: In a clinical trial the Saffron group did significantly better in PMS symptom control than the placebo group.  Saffron is rich in magnesium, vitamin B6, iron and other nutrients that are missing in PMS patients, which explains the effectiveness of this botanical.

Lavender, Motherwort, and Dandelion

Other botanicals: Other botanicals are Lavender, Motherwort, and Dandelion.

Candidiasis

Due to prolonged exposure to high sugar and refined carb intake many women with PMS suffer from candidiasis (chronic yeast infection). Anti-Candida programs help to eradicate Candida overgrowth, which often improves several PMS symptoms.

Mind/body therapy

Cognitive-behavioral therapy helps for depression and anxiety. Hypnotherapy, yoga and biofeedback therapy are also useful methods.

Conclusion

At the present time there is a better understanding of PMS than in the past. Progesterone deficiency and other hormone weaknesses seem to be at the center of this condition. But vitamin and mineral deficiencies also play a role. The healthcare provider should order some baseline blood tests and hormone tests for the patient, including a saliva progesterone level.

Treatment consists of a combination of steps taken simultaneously. The dietary approach comes first: a Mediterranean diet will be beneficial. Next add nutritional supplements. Regular exercise is essential. Finally bio-identical hormone replacement of the missing hormones is necessary.

If there is an underlying chronic candidiasis infection, it needs treatment. The choice of drug would be nystatin. Some botanicals may be helpful, as discussed. When anxiety and depression are important parts of the PMS symptoms, mind/body therapy (such as cognitive therapy etc.) may also be helpful.

The key with PMS treatment is to not give up, but to re-evaluate the condition, if the initial attempt does not bring full relief. By not giving up and using all modalities of treatment the patient will be able to get rid of the condition, eliminate the symptoms of PMS and achieve well being.
